【摘要】 目的观察氯诺昔康用于头颈部恶性肿瘤术后镇痛的疗效及安全性。方法选择头颈部恶性肿瘤手术后要求行术后镇痛患者40例,随机分成L组和F组,每组20例,L组氯诺昔康40mg,F组芬太尼1mg,均用生理盐水稀释至100mL。镇痛泵为福尼亚,背景剂量2mL/h,单次按压剂量1mL,锁定时间15min。手术结束前30minL组给予负荷剂量氯诺昔康8mg,F组于术毕给予负荷剂量芬太尼0.05mg。术毕开始PCIA。观察术后2、4、8、12、24、36和48h的VAS评分、Ramsay评分及不良反应。结果VAS评分两组在各时间点差异无显著性(P>0.05);F组Ramsay评分在术后2和4h明显高于L组(P<0.05),8h以后两组间差异无显著性(P>0.05);F组的不良反应明显多于L组,不良反应的发生率亦高于L组(P<0.05)。结论氯诺昔康不仅具有与芬太尼相近的镇痛效果,而且不良反应少,安全性高,更适用于头颈部恶性肿瘤患者术后镇痛。更多还原

【Abstract】 [Objective] To investigate the effect and safety of lonorxicam on postoperation analgesia to head-cervix malignant tumor. [Methods] 40 patients who administered postoperation analgesia after the operation of head-cervix malignant tumor were divided into 2 groups randomly, Group L and group F, 20 case in each group, 40mg lonorxicam were administered in group L, 1 mg fentanyl in group F, dilute the drug with normal saline to 100 ml respectively. Use Fornia analgesia pump, continuous infusion rate at 2 mL/h, bolus does at 1ml,lockout interval at 15 min. 30min before the finish of the operation, the loading dose 8mg lonorxicam was given in group L; while in group F, the loading dose 0.05 mg fentanyl was given when the operation was finished, Patient-controlled intravenous analgisia started after the operation. The VAS score, the Ramsay score and side effects were recorded at 2, 4, 8, 12, 24, 36 and 48 hours after the operation respectively. [Results] The VAS score in each timepoint in group L was not significantly different from that in group F (P >0.05). The Ramsay scores in postoperative 2, 4 hours were significantly higher in group F than those in group L (P <0.05), and there was no significantly difference between the two groups 8hs after the operation (P >0.05); The side effects in group F were signifcantly higher than that of group L (P >0.05). [Conclusion] The analgesia effect of lonorxicam is similar to that of fentanyl, but lonorxicam is safer, and there is less side effects. Lonorxicam is effective and safe for the postoperation analgesia to head-cervix malignant tumor.更多还原
