准分子激光原位角膜磨镶术前后角膜地形图形态分析

【摘要】 目的了解准分子激光原位角膜磨镶术前后角膜表面的性状变化与疗效关系,以探讨角膜地形图在准分子激光原位角膜磨镶术中的应用价值。方法应用计算机辅助的角膜地形图分析系统(TMS-1 com-puted Anatomy Inc. New York, NY)对70例(137只眼)LASIK术前后角膜地形图的形态进行检测,并比较分析其与手术前后视力及视觉质量的关系。结果术前角膜地形图的形态呈对称和非对称领结形者占81.8%,圆形和椭圆形占13.1%,不规则形占5.1%;术后1个月均匀型占89.8%,蝴蝶结型占8.0%,钥匙孔型占1.5%,中央岛型占0.7%;术后3月均匀型者90.5%,蝴蝶结型占7.3%,钥匙孔型占1.5%,中央岛型占0.7%;切削区形态随时间延长,蝴蝶结型有向均匀型发展的趋势;手术前后角膜地形图形态对术后视力有一定影响。结论LASIK术前后角膜地形图的检查和分析能清楚地显示角膜表面形态变化,有利于个体化实施准分子激光原位角膜磨镶术,获得更完美的视功能。
【Abstract】 To study the relationship between the morphological changes of corneal surface after LASIK and the therapeutic effect, so as to investigate the application value of corneal topography in LASIK. Computer-Assisted Corneal Topogrophy analyzinsg system is used to measure corneal topographic shape, on 137 eye’s of 70 cases with myopia undergone LASIK, and analyze the relationship between these indicates and visual acuity or visual quality. Before LASIK. The percentage of corneal topographic pattern including symmetric bow tie pattern and asymmetric bow tie pattern, round pattern and ellipse pattern, irregular pattern was 81.8%, 13.1% and 5.1%,respectively. One month after LASIK, the percentage of homogeneous pattern, bow tie pattern, keyhole pattern, central island pattern was 89.8%, 8%, 15% and 0.7%, respectively. Three months after LASIK, the percentage of homogenous pattern, bow-tie pattern, keyhole pattern, Central island pattern was 90.5%, 7.3%, 1.5% and 0.7%, respectively. On subsequent follow-up, the bow-tie pattern tended to transform to homogeneous pattern; Corneal topographic shapes before and after LASIK had certain associatiation with visual functions following the surgery. [Conclusions] Examination and Analysis on the corneal topography before and after LASIK can clearly display the morphological changes of the corneal surface morphology, which helps carry out LASIK individually and then achieve perfect refractive correction.
