934例男性尿道感染患者病原学研究

【摘要】 目的:探讨男性尿道感染主要病原菌的种类及分布;评价10种抗生素对各检出菌的作用效果。方法:对临床确诊的男性尿道感染患者的分泌物标本按临床检验操作规程进行分离鉴定,同时对分离的细菌采用K -B法作药物敏感试验。抽取血清测沙眼衣原体IgM抗体。结果:10 18例患者标本,934例检出病原菌,阳性率为91.75 %。其中单菌感染82 .76 % ;两种菌混合感染16 .4 9% ;三种菌共同感染0 .75 %。病原体为淋球菌、非淋球菌细菌、解脲支原体和沙眼衣原体共四类,分别为5 .72 %、73.77%、14 .2 5 %和6 .2 6 %。阳性球菌的感染(6 0 .6 2 % )在尿道炎中仍占重要地位,表皮葡萄球菌检出率最高,占所有鉴定病原菌的2 7.86 %。药物敏感试验结果显示,临床常见9种84 3株细菌对10种抗生素的敏感率低于30 %的菌株占5 5 .5 6 % (5 0 / 90 ) ,表现出极强的耐药性。结论:本地区男性尿道炎主要为非淋菌性尿道炎,细菌是导致男性尿道感染的主要病原菌,沙眼衣原体和解脲支原体感染不可忽视。细菌对抗生素的耐药性日趋严重,不容乐观。尿道感染的患者应兼做多种病原体检测,并通过药敏试验进行合理、科学的治疗才是控制该病的有效途径。

【Abstract】 Objective: To investigate the kinds and disposition of germ caused male urethra infection(MUI) and to estimate the bacteriostasis effect of 10 kinds of antibiotics. Methods: Germ derived from secretion of MUI patients were identified by K-B method and susceptibility test was done. IgM antibody of chlamydia trachomatis was detected. Results: 934 cases were detected from 1018 patients’ samples: positive rate was 91.75%, 82.76% infection caused by one kind of germ, 16.49% by two kinds of germ, 0.75% by three kinds of germ. Pathogens were diplococcus, nongonococcal germ, mycoplasma urealytium and chlamydia trachomatis and the rate was 5.72%?73.77%?14.25% and 6.26% respectively. Gram reaction positive coccobacteria took the important position in urethra infection, and the detection rate of staphylococcus epidermidis was highest (27.86%). Susceptibility test showed that saitivity of 9 common kinds of germs(843 strain) in clinic was lower than 30%, strain and appeared hadro drug resistance. Conclusions Nongonococcal urethritis was the main cause of MUI in this territory.MUI was mainly caused by germ, secondly by chlamydia trachomatis and mycoplasma urealytium. Drug resistance of germ was more and more serious, and manifold diction and drug resistance test was necessary to the detection and therapia for MUI.
