【摘要】 目的探讨老年人在合并华支睾吸虫感染时胆道疾病的外科治疗特点。方法手术治疗胆道疾病合并华支睾吸虫感染老年患者85例,其中包括胆道结石62例(729%),单纯吸虫引起的梗阻11例(129%),胆道恶性肿瘤12例(141%)。合并急性胰腺炎5例(59%),其中3例由胆道结石引起,1例由吸虫梗阻引起,1例由胆管癌引起。手术以胆囊切除加胆总管探查和内镜下十二指肠乳头扩约肌切开术(EST)为主。结果良性病变均于术后痊愈出院。12例恶性病变中2例在围手术期内死亡,10例生存期2～6个月。结论老年人华支睾吸虫感染后更易引起相关的胆道外科疾病,外科治疗的同时应注意对吸虫本身的处理。更多还原

【Abstract】 Objective To investigate the surgical treatment for biliary tract diseases complicated with clonorchiasis sinensis in elderly patients.Methods Clinical data from 235 patients with diseases of biliary tract underwent surgical intervention with clonorchiasis sinensis 85 elderly patients.In all elderly patients,62(72.9%) with cholecystolithiasis and(or) calculus of bile duct,11(12.9%) with biliary obstruction only caused by fluke,12(14.4%) with biliary malignant tumor.5(5.9%,3 caused by calculus of biliary tract,1 caused by obstruction of fluke,1 caused by malignant tumor) complicated with acute pancreatitis.Surgical treatments mainly included cholecystectomy and(or) exploration of common bile duct and(or) endoscopic sphincterotomy(EST).Results All cases of benign diseases were fully recovered after operation.2 of 12 patients with malignant tumor died during the perioperative period,and 10 survived from 2 months to 2 years.Conclusion Clonorchiasis sinensis is easy to cause diseases of biliary tract in elderly patients.In surgical procedure,the treatment for clonorchiasis sinensis should be emphasized.更多还原
