正常人腕骨三维运动学研究方法

【摘要】 目的探讨正常人腕骨三维运动学在体研究方法。方法对10例正常人的右手腕,分别于中立位,掌屈、背伸各30°、60°位,桡、尺偏各10°、20°位建立螺旋CT图像,在计算机手术模拟系统内执行三维配准及三维截骨。以桡骨远端为参照骨建立三维坐标系统,编写桡骨远端及腕骨的质心、转动惯量、有限螺旋轴运算等程序。结果在该系统上构建出正常腕骨的立体结构模型,成功进行每块腕骨的三维切骨,并计算出腕骨及桡骨远端的质心及转动惯量。有限螺旋轴表达腕骨的平移及旋转运动方便可靠,三维测量方法项目全面、操作简捷、准确可靠。结论该研究为腕骨三维运动学提供了实用有效的技术手段和全新的科学方法,制定了一定的理论及技术参考。

【Abstract】 Objective To study the method of normal three-dimentional carpal kinematics in vivo. Methods The right wrists of 10 healthy volunteers were imaged by spiral CT at the neutral position, at 30° flexion and 60° extension, and at 10° ulnar deviation and 20° radial deviation. The bone 3D registration and 3D segmentation all can be performed in our computer-operated simulation system. A 3D coordinate system was then estabilished which was derived from the distal radius to estabilish the formality operation of centroid and moments of inertia and finite helical axis of the distal radius and carpal. Results By means of our system and related methods,the life-like three-dimensional model of carpal anatomic structures was reconstructed for qualitative observation and quantitative analysis. The carpal osteotomy procedures and the centroid and moments of inertia of distal radius and carpal were successfully simulated on the system platform. The 3-D measuring method created by us possesses features of accurate, precise, easy-operating. Conclusion The clinical application of the system indicates that the computerized system can provide some theoretical basis and technical references for the research of 3-D carpal kinematics in vivo.
