改良指动脉逆行岛状皮瓣的临床应用
Modified distally based digital artery island flap: clinical application
【摘要】 目的 探讨改良指动脉逆行岛状皮瓣的静脉回流和重建皮瓣感觉功能的手术方法。方法 设计在指动脉逆行岛状皮瓣血管蒂上加带皮肤蒂 ,同时将皮瓣上的感觉神经与指神经终末支缝合 ,临床应用 10例。结果 10例皮瓣全部成活 ,术后未出现静脉回流障碍。随访 3~ 2 2个月 ,皮瓣质软 ,两点分辨觉在 4mm以内。结论 改良的指动脉逆行岛状皮瓣比常规同类皮瓣更安全可靠 ,疗效满意。
【Abstract】 ObjectiveTo introduce a modified distally based digital artery island flap with improved venous drainage and sensation restoration. MethodsA 3mm~4mm wide skin pedal was included around the vascular pedicle while harvesting the distally based digital artery island flap. A cutaneous nerve was included in the flap and coapted with the distal end of the digital nerve. This modified flap was used to cover skin defect of the thumb and fingers in 10 cases. Postoperative follow-up ranged from 3 to 22 months.ResultsAll the flaps survived with no signs of venous congestion. The flaps were soft and sensate, with 2 point discrimination within 4 mm.ConclusionModified distally based digital artery island flap is safer and more reliable than the standard one.
