磁共振梯度回波T2*成像诊断急性脑出血的意义
Diagnostic values of gradient-echo T2*-weighted image for hyperacute cerebral hemorrhage
【摘要】 目的评价磁共振梯度回波T2成像(GRET2WI)诊断急性脑出血的意义。方法发病1~24h的21例急性脑出血患者在行CT检查后,行GRET2WI检查及常规MRI检查,比较其诊断价值。结果21例出血灶在GRET2WI均表现为边界清楚的极低信号,或表现为边界清楚的低信号环,内部为略高信号或低信号区内混杂小点、斑片状高信号,使脑出血病灶显示非常清楚。6例在基底节区,同侧或对侧丘脑发现有1~7个直径2~5mm的圆形、斑点状的极低信号,为GRET2WI检测出的脑微出血。结论应用GRET2WI技术可以早期准确地诊断脑出血,可以全面了解患者颅内损害情况。
【Abstract】 Objective To evaluate the role of gradient-echo T*2-weighted image (GRE-T*2WI) in diagnosis of hyperacute cerebral hemorrhage. Methods GRE-T*2WI was performed in 21 cases of cerebral hemorrhage within 1—24 hours after symptom onset, and the results were compared with routine CT and MRI to learn which one is more effective. Results Parenchymal hemorrhage appeared as homogeneous patchy hypointension or in homogeneous hypointensity with punctuate hyperintensity on GRE-T*2WI. Among 21 patients, parenchymal hemorrhage appeared clearly. There were several dots or patchy hypointension with a diameter about 2—5 mm in ganglia and thalamus areas on 6 patients’GRE-T*2WI, which were cerebral microbleeding diagnosed by GRE-T2*WI. Conclusion GRE-T2*WI should be a much more sensitive method than CT or the routine MRI and could get more message, which might be used to diagnose hyperacute cerebral hemorrhage.
