慢性前列腺炎病原学初步研究

【摘要】 目的探讨病原微生物在慢性前列腺炎发病中的作用。方法随机选择慢性前列腺炎患者101例,有慢性前列腺炎症状和(或)前列腺液WBC计数>10/HP;健康对照68例。对169例受试对象的中段尿、前列腺液(EPS)/按摩后尿病原微生物进行检测,组间阳性率比较采用χ2检验。结果慢性前列腺炎组及健康对照组中段尿细菌培养阳性率分别为5.0%、4.4%,EPS/按摩后尿细菌培养阳性率分别为37.6%、39.7%,EPS/按摩后尿性病系列(STD)病原微生物检测阳性率分别为35.6%、35.3%。2组中段尿、EPS/按摩后尿病原微生物捡出率差异无统计学意义,且前列腺炎组不存在随EPS中WBC计数增加EPS/按摩后尿培养结果阳性率增加的趋势。结论慢性前列腺炎组与健康对照组EPS/按摩后尿培养的病原微生物种类及阳性率差异无显著性。细菌及支原体、衣原体可能不是慢性前列腺炎的病原体,而是一种移生或伴生现象。

【Abstract】 Objective To investigate the effects of pathogenic organisms on chronic prostatitis.Methods A total of 101 patients with chronic prostatitis(symptomatic and/or WBC count >10/HP in prostatic secretion) and 68 healthy controls underwent tests of midstream urine culture,WBC counting of expression of prostate secretion(EPS),EPS/post-prostatic massage urine(PPMU) culture,and sexually transmitted disease(STD) test of EPS/PPMU.The positive rates of pathogens were compared between the 2 groups using χ~2 test.Results The positive rates of bacterial culture with midstream urine in patient group and controls were 5.0%(5/101) and 4.4%(3/68),respectively;and the rates with EPS/PPMU were(37.6)%(38/101) and 39.7%(27/68),respectively.There was no significant difference in the positive rates between the 2 groups(P>0.05).The positive rate of STD bacterial culture with EPS/PPMU in patient group was 35.6%(36/101);and that in controls was 35.3%(24/68).Similarly,no significant difference was found between the 2 groups(P>0.05).In addition,no significant correlation was found between the positive EPS/ PPMU cultures and WBC counts in EPS in patient group.Conclusions There is no significant difference in species and prevalence of pathogenic microorganisms in urine and EPS/PPMU between chronic prostatitis patients and healthy controls.It is suggested that microorganisms may not be the cause of chronic prostatitis and that the prostate is often colonized by commensal urethral bacteria in men with or without chronic prostatitis.
