18F-FDG符合线路显像在头颈部恶性肿瘤诊断中的应用

【摘要】 目的 评价1 8F 脱氧葡萄糖(FDG)符合线路显像(DHTC)诊断头颈部恶性肿瘤的应用价值。方法 头颈部恶性肿瘤患者5 5例,其中鼻咽癌34例(初发12例,放疗后2 2例)、其他头颈部癌术后13例和原因不明的颈部转移瘤8例,进行1 8F FDGDHTC。结果 ①1 8F FDGDHTC诊断原发头颈部恶性病灶的灵敏度、特异性和准确性分别为90 5 % ( 19 2 1例) ,80 8% ( 2 1 2 6例)和85 1% ( 4 0 4 7例) ;诊断转移瘤的灵敏度和特异性分别为93 8% ( 30 32例)和86 7% ( 13 15例)。②8例淋巴结转移瘤患者中,4例颈外见异常单个1 8F FDG浓聚灶,为原发病灶;1例显像阴性;3例见多发性1 8F FDG异常浓聚,未能定位原发病灶。结论 1 8F FDGDHTC诊断原发头颈部恶性病灶和寻找颈部转移瘤的原发灶有较大的临床价值

【Abstract】 Objective To assess the value of 18 F fluorodeoxyglucose (FDG) SPECT in the diagnosis of neck and head malignancy. Methods Fifty five patients with neck and head malignancy were underwent 18 F FDG SPECT imaging and registration with integrated CT. Among them, 34 cases were with nasopharyngeal carcinoma (12 primary cases and 22 post radiotherapy cases), 13 cases with other kinds of postoperative malignancy and 8 cases with lymph node (LN) metastases in the neck and head. Results ① 18 F FDG SPECT imaging showed that the sensitivity, specificity and accuracy for diagnosing head and neck malignancy were 90.5%(19/21), 80.8%(21/26) and 85.1%(40/47), respectively. The sensitivity and specificity for detecting metastases were 93.8%(30/32)and 86.7%(13/15). ②The primary lesions were found in 4 of 8 patients with neck LN metastases, and the primary lesions in the other 4 cases were not identified. Conclusion 18 F FDG SPECT imaging is of higher accuracy in the diagnosis of neck and head primary malignancy, and of great value for detecting clinically unknown primary lesions with neck LN metastases.
