前列腺癌与良性前列腺增生症的MR动态增强与血管生成的相关性研究
Dynamic contrast-enhanced MR of the prostatic cancer and benign prostatic hyperplasia: correlation with angiogenesis
【摘要】 目的 研究MR动态增强对前列腺癌与良性前列腺增生的诊断价值 ,并初步探讨MR动态增强表现特征与血管生成的相关关系。方法 对 32例前列腺癌和 4 0例良性前列腺增生 (BPH)的患者进行常规MR平扫和动态增强扫描 ,观察病灶的动态增强MRI特征并绘制信号强度 时间曲线(SI T曲线 ) ,SI T曲线分成 3型 :Ⅰ型为信号强度早期增高后仍持续增高 ;Ⅱ型为信号强度早期增高后出现平台期 ;Ⅲ型为信号强度早期增高后出现下降期。获得感兴趣区的病理标本 ,并进行苏木素 伊红 (HE)染色及免疫组织化学血管内皮生长因子 (VEGF)和微血管密度 (MVD)测定 ,比较分析动态增强参数及曲线类型与VEGF、MVD的相关关系。结果 前列腺癌MRI动态增强的早期强化参数开始时间、最大信号强度及早期强化率与BPH不同 (P分别为 <0 0 1、0 0 5、0 0 1 ) ,但两者之间有部分重叠。动态增强MRI中后期的强化特征即SI T曲线类型中 ,前列腺癌组 :Ⅰ型曲线 3例、Ⅱ型曲线 8例、Ⅲ型曲线 2 1例 ;BPH组 :Ⅰ型曲线 1 8例、Ⅱ型曲线 1 9例、Ⅲ型曲线 3例 ,前列腺癌以Ⅲ型曲线为主 ,Ⅱ型曲线既可见于前列腺癌又可见于BPH ,Ⅰ型曲线多见于BPH。前列腺癌与BPH的SI T曲线类型的差异具有统计学意义 (P <0 0 0 1 )。前列腺癌的VEGF和MVD的测定水平高于BPH (
【Abstract】 Objective To evaluate the role of dynamic contrast-enhanced magnetic resonance imaging (MRI) in the diagnose of prostatic cancer and benign prostatic hyperplasia (BPH), and to determine the correlation between dynamic MRI findings with angiogenesis.Methods Thirty-two cases of prostatic cancer and 40 cases of BPH underwent dynamic contrast-enhanced MRI.All the patients in this study were diagnosed by histopathology.The results of dynamic contrast-enhanced MRI were evaluated by early-phase enhancement parameters and time-signal intensity curves (SI-T curves), and the curves were classified according to their shapes as type Ⅰ, which had steady enhancement; type Ⅱ, plateau of signal intensity; and type Ⅲ, washout of signal intensity.The pathologic specimens of region of interest (ROI) were obtained, and HE staining, immunohistochemical vascular endothelial growth factor (VEGF), and microvessel density (MVD) measurements were performed.The relationships among dynamic contrast-enhanced MRI features, VEGF, and MVD expression were analyzed.Results In the early-phase enhancement parameters of dynamic contrast-enhanced MRI, onset time,maximum signal intensity, and early-phase enhancement rate differed between prostatic cancer and BPH(P<0.01, 0.05, 0.01), but there were some overlaps between them.The intermediate and late post-contrast periods were characterized with the lesion SI-T curves.The SI-T curve of prostatic cancer was mainly type Ⅲ (21 cases).Type Ⅱ could be seen in both prostatic cancer (8 cases) and BPH (19 cases).Type Ⅰ most appeared in BPH (18 cases).The distributions proved to have significant difference (P<0.001).The mean VEGF and MVD level of 32 prostatic cancer patients were significantly higher than those of 40 BPH patients (P<0.001).MVD level of prostatic cancer and BPH showed an association with VEGF level (P<0.01).The maximum signal intensity and early-phase enhancement rate in both prostatic cancer and BPH showed an association with MVD expression (P<0.05 or 0.01).Conclusion Dynamic contrast-enhanced MRI are useful in the diagnose and differential diagnose of prostatic cancer and BPH.The characters of dynamic contrast-enhanced MRI are closely related to VEGF and MVD level.As a noninvasive method, dynamic contrast-enhanced MRI has a potential role in evaluating the degree of angiogenesis of prostatic cancer and BPH.
