应用核素显像对胆囊炎胆囊运动功能的研究
Study of gallbladder motor function in cholecystitis by scintigraphy
【摘要】 目的应用核素显像评价生理及胆囊炎条件下胆囊的运动功能,发现其各自特点,探讨胆囊炎的病因及其对胆囊功能的影响,以有助于预防疾病和确定治疗方案。方法对9例正常自愿者及31例胆囊炎病人行核素显像检查,应用感兴趣区(ROI)技术,得到时间活性曲线(TAC),计算胆囊排空分数(GBEF)及T1/2,胆囊排泄率(ER),计算胆汁流入胆囊和肠道的百分比。各组数据正常与疾病进行比较。结果慢性胆囊炎病人的GBEF与正常组无明显差异,ER有显著性差异,T1/2和胆肠分配比有显著差异。其中GBEF正常者的上述差异更为显著。结论胆囊炎病人的胆囊运动功能障碍;胆囊结石不仅是胆汁生化和胆囊收缩运动异常,胆总管、Oddi括约肌功能状态也是重要因素;应用核素显像可以反映胆囊的生理过程,可用于确定胆囊疾病的治疗方案。
【Abstract】 Objective To study the gallbladder motor function in physiological and cholecystitis conditions by scintigraphy to find its features, investigate the pathogenetic factors and action to help with prevention of the disease and determination of treatment plans. Methods Scintigraphy was performed in 31 patients with cholecystitis and 9 controls. The technique of ROI was used to obtain time-active curve (TAC) and calculate GBEF, T_ 1/2 and ER and the percentage of bile flowing into the gallbladder and intestine. Results There was no significant difference in GBEF between the patients and controls. However, there were marked differences in ER, T_ 1/2 and percentage of bile flowing into the gallbladder and intestine between the 2 groups. When the GBEF was over 50%, the differences between the controls and patients were more notable. Conclusions The gallbladder motor function is abnormal in the patients with cholecystitis. Besides biochemical changes in bile and abnormal contraction motor, the functional state of bile duct and Oddi’s sphincter are also important factors for gallbladder lithogenesis. The scintigraphy is sensitive to determine the treatment plans for gallbladder disease in clinical practice.
