二尖瓣运动装置功能结构参数的三维超声心动图筛选

【摘要】 目的运用三维超声心动图技术筛选出能够诊断二尖瓣运动装置病变的三维功能结构参数。方法以二尖瓣前组乳头肌长度(APM)、后组乳头肌长度(PPM)、前后瓣膜至前组乳头肌边缘腱索长度(CTaa,CTpa)、前后瓣膜至后组乳头肌边缘腱索长度(CTap,CTpp)、瓣环面积及代表瓣膜运动幅度的α角和β角作为待筛选的二尖瓣装置功能结构参数。运用TomTec三维超声图像处理工作站(Echo View4.2)分别测量20例患有先天性心脏病二尖瓣关闭不全儿童和20例正常儿童的二尖瓣装置功能参数数值。三维重建H切面测量APM、CTaa和CTpa,三维重建S1a切面测量瓣环面积,三维重建后组乳头肌显示切面测量PPM、CTap和CTpp。测得的参数数值经EXCEL 2000内置软件包分析。结果两组CTaa、瓣环面积及α角、β角间差异有显著性意义(均P<0.05)。病例组测得的瓣环面积显著大于对照组;CTaa明显短于正常;前后瓣膜开放幅度较正常小。结论三维超声心动图测量二尖瓣装置功能结构参数CTaa、α角、β角和瓣环面积可能对二尖瓣装置异常的评估有一定的意义。

【Abstract】 Objective To screen the functional structure parameters of mitral apparatus abnormality using three-dimensional echocardiography(3DE).Methods The length of the anterior papillary muscle(APM),and the posterior papillary muscle(PPM),length of the chord from anterior valve or posterior valve to anterior papillary muscle(CTaa,CTpa),length of the chord from anterior valve or posterior valve to posterior papillary muscle(CTap,CTpp),mitral annulus’ area,the angle between mitral valve annulus and anterior leaflet(α) and posterior leaflet(β) served as functional structure parameters of mitral apparatus for further screening.All the parameters were measured in 20 children with valve incompetence by 3DE and compared with those in 20 age and sex matched normal children.H,S1a and the posterior papillary muscle plane of 3DE cutting planes were applied for measurements.The values were analyzed by EXCEL 2000 software.Results There were significant differences between two groups in CTaa,area,α angle and β angle(all P<(0.05)).Conclusions Three dimensional measurement of CTaa,area,α angle and β angle could have definite value for evaluating abnormal mitral apparatus.
