超声预测胎儿双肩径方法的研究
Methods for fetal bishoulder distance measured by ultrasound
【摘要】 目的探讨超声测量胎儿双肩径的方法及临床应用价值。方法于分娩前3d对200例单胎孕37~42周的孕妇行超声检查,测量胎儿双肩径、单侧锁骨长、单侧肩胛骨长,并与生后24h内的新生儿各径线比较,同时将超声测量的各径线值分别与新生儿双肩径作相关性分析,比较胎儿锁骨长、肩胛骨长及新生儿双肩径等参数与巨大儿的关系。结果不同性别胎儿的锁骨长度及肩胛骨长度不同。超声测量的锁骨长及肩胛骨长与生后测值有良好的符合性,但出生前后的双肩径测值差异有显著性意义。胎儿单侧锁骨长及肩胛骨长与新生儿双肩径有良好的相关性,且以锁骨长的相关性更好,并得出了以锁骨长为指标的预测胎儿双肩径的线性方程。结论超声测量胎儿锁骨及肩胛骨长度,可作为预测胎儿双肩径的一种可行的方法,指导临床医生选择适当的分娩方式,减少肩难产的发生。
【Abstract】 Objective To evaluate the methods and predict the value of fetal bishoulder distance measured by ultrasound. Methods A total of 200 women at 37-42 weeks′ gestation, delivering a singleton infant and having an ultrasound examination within three days before delivery were studied. The clavicula, scapula and bishoulder distance were measured by ultrasound and compared with newborn bishoulder distance, which were measured within 24 hours after birth. Their correlation with newborn bishoulder distance was also analyzed. Results Mean values of clavicula and scapula distance were different by sex. Each diameter(except for bishoulder distance) showed no significant difference from that measured after birth. Fetal clavicula and scapula distance had positive correlation with newborn bishoulder distance, and the correlation of fetal clavicula distance was supper to that of fetal scapula distance. There was a linear connection between fetal clavicula and newborn bishoulder distance. Conclusions Fetal clavicula and scapula distance measured by ultrasound could predict fetal bishoulder distance,which helps guide in delivery choice and reduce difficult delivery by bishoulder distance.
