激素与非激素起搏电极术后房室波振幅及阻抗的变化
The Change of Atrial and Ventricular Amplitude and Impedance for Steroid and Non-steroid-eluting Lead After Implantation.
【摘要】 为观察激素与非激素起搏电极术后A、V波振幅及阻抗的变化规律,对40例接受心脏起搏器治疗的患者分别于术中;术后1,2周及1,2,3,6个月在平静状态下测定并比较以上起搏参数。结果:激素电极术后1个月A波振幅明显升高;V波无明显变化;非激素电极A、V波振幅均无明显变化。激素与非激素电极A、V波各时间点所测之值比较,无显著差异(P>0.05)。不管是激素电极,还是非激素电极术中所测电极阻抗,均较术后各时间点所测之值高(P<0.05)。激素电极术后阻抗比较稳定,非激素电极术后有逐渐降低的趋势。非激素电极各时间点所测阻抗明显高于激素电极阻抗(P<0.01)。结论:起搏电极置入术后,A波振幅于平静时偶尔出现较明显波动,但V波振幅相对稳定;电极阻抗于术后1周时开始明显降低,激素电极阻抗保持稳定,而非激素电极阻抗则术后波动较大。
【Abstract】 To observe the regularity of changes with time of A wave, V wave and pac ing lead impedance, parameters were measured respectively during operation and 1 st, 2and week, 1 st, 2 nd, 3 rd and 6 th month postoperation in 40 patients who underw ent implantation of pacemakers. Results: For patients who were implanted with steroid-eluting leads, A wave amp litude increased considerably and the V wave amplitude did not change significantly. The amplitude of A wave and V wave for patients with non-steroid-eluting lead did not change significa ntly during and postoperative. The amplitude of A wave and V wave were not significantly different between the steroid-eluting leads and the non-steroid-eluting leads during and postoperat ive (P>0.05). The impedance of steroid-eluting leads and non-steroid-eluting leads decreased considerably after operation (P<0.05) .The impedance of steroid-eluting leads remained stable after operation. The impedance of non-steroid-eluting leads gradually decreased after operation. The impedance of non-steroid-eluting leads were significantly higher than that of the steroi d-eluting leads (P<0.01). Conclusions: A wave amplitude may change obviously, but V wave rem ains relatively stable within 6 months after implantation; For a steroid-eluting lead the imped ance decreases significantly in one week after implantation, and it remains stable in 6 months, while for a non-steroid-eluting one itmay vary obviously within the normal range.
