【摘要】 目的探讨奥氮平合并舒必利治疗难治性精神分裂症的疗效及不良作用。方法对97例难治性精神分裂症患者按CCMD-Ⅲ标准及难治性标准确诊后,随机分成三组,分别用舒必利、奥氮平、舒必利合并奥氮平治疗6个月,在治疗前及治疗后2,4,6个月用BPRS、SANS、SDSS、CGI量表评定疗效。结果研究结果显示三种方法治疗难治性精神分裂症6个月后均有效,舒必利、奥氮平、舒必利合并奥氮平三组有效率分别为59.4%、30.0%、37.9%。三组在治疗后2,4,6个月BPRS、SANS评分与治疗前相比较均有显著下降,差异有非常显著性(P<0.01),舒必利合并奥氮平治疗有效率与另两组比较差异有非常显著性(P<0.01),治疗后2,4,6个月BPRS、SANS减分率与另两组比较差异具有显著性(P<0.05)。舒必利合并奥氮平的不良反应明显减少,主要为窦性心动过速、EPS、便秘、体重增加。结论三种方法治疗难治性精神分裂症6个月均有效,舒必利合并奥氮平较单一用药更有效,不良反应少。更多还原

【Abstract】 ObjectiveTo evaluate the efficacy and side effect of olanzapine combined with sulpiride therapy in refractory schizophrenia. Methods97 patients met CCMD-III diagnostic criteria and the standard of refractory were divided into three groups randomly, and then separately assigned to 6 months treatment with sulpiride, olanzapine or sulpiride combined with olanzapine. Before treatment and after 2 months, 4 months and 6 months treatment, effectiveness was evaluated by BPRS, SANS, SDSS and CGI. ResultsThree therapys were effective on refractory schizophrenia after 6 months treatment,after treatment with olanzapine combined with sulpiride, the effective rate was 59.4%, which showed marked statistical difference ( P <0.01) from sulpiride monotherapy (the effective rate was 30%) and olanzapine monotherapy (the effective rate was 37.9%). After 2 months, 4 months,and 6 months treatment with three therapys,the scores of BPRS and SANS were decrease and statistical different from before treatment( P <0.01),the reducing rate of BPRS and SANS which treatment with oanzapine combined with sulpiride were statistical different from sulpiride monotherapy and olanzapine monotherapy ( P <0.05). Compared with olanzapine and sulpiride monotherapy, the side effects of olanzapine combined with sulpiride therapy were decreased obviously. The main side effects were sinus tachycardia,EPS,coprostasis and weight gain.ConclusionThree therapys were effective on refractory schizophrenia after 6 months treatment. olanzapine combined with sulpiride therapy in refractory schizophrenia shows better efficacy than monotherapy and shows less side effects.更多还原
