铜蒸汽激光光动力疗法治疗微静脉畸形238例分析
Clinical analysis of 238 cases of venular malformation treated by copper vapor laser photodynamic therapy
【摘要】 目的:总结铜蒸汽激光光动力治疗微静脉畸形的疗效,探讨影响疗效的因素。方法:1997年9月~2003年6月间,采用铜蒸汽激光光动力治疗微静脉畸形238例,其中粉红色(Ⅰ型)20例、浅红色(Ⅱ型)44例、深红色(Ⅲ型)99例、紫红色(Ⅳ型)51例、增厚或结节型(Ⅴ型)24例。结果:随访6个月~4a,治愈68例(28.57%),显效76例(31.93%),有效87(36.55%)例,无效7例(2.94%),其中3例产生瘢痕。结论:铜蒸汽激光光动力疗法具有选择性破坏病灶畸形血管,不损伤正常皮肤的特点,只要治疗技术掌握恰当,选择合适的治疗年龄,多数浅薄型病变能够治愈或获得显著疗效,增厚型病变能明显变薄、颜色变浅、病变发展变缓。
【Abstract】 PURPOSE: To summarize the clinical effects of copper vapor laser photodynamic therapy (PDT) for venular malformation and analyze the relationship between therapeutic effects and types of lesion, age, and time of treatment. METHODS: From September 1997 to June 2003, 238 venular malformation cases were treated by PDT. Among them, 20 cases had pink lesions(type I), 44 cases had red lesions (typeⅡ), 99 cases had dark red lesions( type Ⅲ),51 cases had purple lesions (type Ⅳ), 24 cases had nodular or thickened lesions (type Ⅴ). RESULTS: All the patients were followed from six months to four years, 68 cases were cured, 76 cases greatly improved, 87 cases improved, 7 cases had no effects, 3 cases formed scars. CONCLUSION: Copper vapor laser PDT can selectively destroy the venules in the lesion without any damage to skin. If this technique was applied properly at the proper age of the patients, most thin lesions can heal or greatly improve, thick lesions can become thinner, lesion color can become lighter, and lesion aggression can be slowed.
