【摘要】 目的评价高强度聚焦超声(HIFU)对浅表部位恶性肿瘤的治疗效果和副作用。方法31例原发灶已经切除,在身体浅层发生转移或复发的肿瘤,予以HIFU治疗,超声发射电功率为1000～2000W,频率为1.04MHz,焦域大小为6mm×6mm×10mm。结果完全缓解(CR)1例,部分缓解(PR)19例,稳定(SD)9例,恶化(PD)2例,有效率93.5%;随访5～31个月,3例失访,患者的中位生存期16.7个月,2年生存率为39.3%。副作用皮肤深II度烫伤2例,浅II度烫伤5例,无其它不良反应。结论HIFU对浅表部位恶性肿瘤有明显的治疗作用,其副作用轻微,可以作为手术后肿瘤复发或转移的补救措施。更多还原

【Abstract】 Objective To explore the clinical effect of HIFU on the treatment of malignant tumours in superficial areas and the side effects.Methods There were 31 cases with malignant tumours in superficial areas including stromal sarcoma in the abdominal wall (4 cases);incisional implanted carcinoma after surgery of urinary Bladder(7) and colonal carcinoma(3);superficial lymphnode metastases in supraclavicular node(9),submandibular(3)and inguinal region(5).All the 31 cases were treated with HIFU.The power of the HIFU was 1 000-2 000 W,the frequency was 1.04 MHz,the size of the focal zone was 6 mm×6 mm×10 mm.Results After HIFU treatment,complete remisson (CR)was in 1 case,partial remisson(PR)(19 cases),stable disease(SD)(9 cases)and progression disease(PD)(2 cases).The effective rate was 93.5%.The cases were followed up for 5 to 27 months except three of them.The patient’s medium survival time was 16.7 months.2 year-survival rates were 39.3%.The side effects were deep burn with degreeⅡin 2 cases and superficial burn of degreeⅡin 5.Conclusions HIFU is effective in the treatment of malignant tumours and metastastic foci in superficial soft tissues with mild side effects.HIFU may be used as an adjuvent treatment for malignant tumours resected or metastatic foci,or implanted tumours such as stromal sarcoma in the superficial soft tissue or lymphnodes.更多还原
