低频超声对猪脑内血肿助溶作用的实验研究
Effect of Low-frequency Ultrasound-enhanced Fibrinolysis in Intracerebral Hematoma:A Porcine Model Experiment
【摘要】 目的 评价低频超声对猪脑内血肿助溶作用的有效性和安全性。方法 2 4只实验猪采用自体血二次注入法制作脑出血模型 ,并于造模后 6 h随机分组实施治疗。治疗组 用低频超声(L FUS) +尿激酶 (UK) +微泡 (MB) ,治疗组 用 L FUS+UK,治疗组 单用 U K,对照组注入生理盐水。治疗后开颅取脑 ,用计算机图像分析系统计算各组脑内血肿残余量。同时用光、电镜观察血肿周围脑水肿程度和脑组织超微结构变化。结果 各治疗组残余血肿量较对照组有明显减少 (P<0 .0 1) ;治疗组 和治疗组 残余血肿体积均较治疗组 有明显减少(P<0 .0 5 ) ,但前两组比较差异无显著性 (P>0 .0 5 ) ;光、电镜观察血肿周围脑组织形态学改变各组在程度上无明显差别。结论 L FUS对脑内血肿有助溶作用 ,且对脑组织无病理性损害。
【Abstract】 Objective To evaluate the efficacy and safety of enhanced fibrinolysis by low-frequency ultrasound (LFUS)in intracerebral hematomas in porcines.Methods A total of 24 pigs with intracerebral hematoma by double injections of autologous blood were divided randomly into 4 groups (n=6)in each group.Different treatment were carried out 6 hours after hematoma created.In groupⅠ:clots were lyseed with LFUS+urokinase(UK)+microbubbles(MB);In groupⅡ:hematomas were treated with LFUS+UK;group Ⅲ:with UK;control group:with saline injection.In all the 4 groups,clots were aspirated after the treatment.Pigs were sacrificed 24 hours after the establishment of the model,and brains were fixed with formaldehyde.The residual hematoma volumes were determined on coronal sections using computer-assisted morphometry.Pathological and ultrastructural changes of perihematoma were observed under light microscope and electron microscope.Results The residual hematoma volumes of the treatment groups were decreased significantly than that of the control group(P<0.01).The residual hematoma volumes of treatment groupⅠand Ⅱwere decreased significantly than that of the treatment groupⅢ(P<0.05).but there was no difference between the former two groups(P>0.05).Light microscopic examination demonstrated that the degree of perihematoma edema is similar.Under the electron microscope,no significant difference of ultrastructural changes of perihematoma was found among the 4 groups.Conclusions LFUS can enhance fibrinolysis,so it is an effective and safe method for the treatment of intracerebral hemorrhage.
