自身肺段温差及其引导下肺活检诊断肺癌临床研究

【摘要】 目的探讨自身肺段温差及其引导下肺活检诊断肺癌的临床价值。方法以自制的肺癌探测议,测试50例951个正常肺段温度,确定人体肺段正常温度值。对比不同肺部疾患局部与自身健康肺段温度差,确定温差诊断肺癌的阳性参考标准。对比肺癌诊断中温度差阳性率与病理符合率,对比温差引导下肺活检与常规肺活检肺癌诊断符合率。结果确定人体肺段正常温度为36.6~37.7(36.89±0.37)℃。发现良性肿瘤、肺结核、支气管肺部感染病灶局部与自身健康肺段温差均<1.5℃,肺癌病灶局部与自身健康肺段温差均≥1.8℃。以≥1.8℃为肺癌诊断阳性标准,30例已确诊肺癌温差值均为阳性,与病理诊断符合率达97%,对55例可疑周围型肺癌常规肺活检确诊36例(65.5%),温差引导下肺活检确诊51例(92.7%),2种方法差异有显著性(P<0.01)。结论自身肺段温差及其引导下肺活检为提高肺癌尤其周围型肺癌的诊断正确率提供了一条新途径,有一定临床价值。

【Abstract】 abjective To study the chinical value of diagnosing lung cancer through oneself lung segment temperature difference and lung biopsy under its guidance. methods Adopting contrastive testing method with the help of self-made lung segment temperature difference detector, authors detected normal lung segment temperature difference of 50cases(951 lung segments) for confirming normal temperature difference reference value, and comparing temperature difference of lung segment of different diseases to self health lung segment for confirming positive reference standard of diagnosing lung cancer by temperature difference. And we have compared positive rate of temperature difference to pathological coincedence rate, compared diagnostic coincedence rate of lung biopsy led by temperature difference to conventional lung biopsy. Results Normal temperature of human lung segment is 36.6~37.7(36.89±0.37)℃. Temperature difference of lung segment between benigh tumor, pulmonary tuberculosis, broncho-pulmonary infection in local focus and self health lung segment all less than 1.5℃, the temperature difference between local focus of cancer and self health lung segment all more than 1.8℃. According to 1.8℃ positive standard 30 confirmed lung cancer cases are all positve, the coincedence rate with pathological diagnosis is 97%. 36(65.5%) of 55 cases suspected peripheral bronchogenic carcinoma are confirmed by conventional lung biopsy, 51(92.7%) of 55 cases are confirmed under guidance of temperature difference, The difference of 2 methods is significant(P<0.01). Conclusion Oneself lung segment temperature difference and lung biopsy under its guidance provide a new way for raising accuracy diagnosis rate especially peripheral lung cancer, it has a certain clinical value.
