【摘要】 目的了解人乳头瘤病毒(HPV)与妇女大肠癌的发生及病理组织学的关系。方法用多重引物PCR方法检测45例妇女大肠癌标本和35例对照组的HPVDNA,并与病理组织学结果进行比较。结果大肠癌总HPV阳性率为42.2%,其中HPV16为24.4%,与对照组比较差异有显著性(P<0.05)。总HPV和HPV16的阳性率,高分化腺癌组为41.2%和17.6%,中分化组为38.5%和23.1%,低分化组为46.7%和33.3%,差异无显著性(P>0.05),HPV6/11及HPV18型阳性率各组比较,差异均无显著性(P>0.05)。总HPV阳性率,印戒细胞癌组(50%)略高于管状腺癌、黏液腺癌和乳头状腺癌组(P>0.05);HPV6/11阳性仅见于管状腺癌组(13.0%,P>0.05)。HPV16型感染则存在于各型癌细胞中(均超过20%),恶性程度较高的黏液腺癌和印戒细胞癌中阳性率分别达33.3%和50.0%,其中印戒细胞癌显著高于管状腺癌和乳头状腺癌(P<0.05)。结论HPV感染,尤其是HPV16与妇女大肠癌有较密切的关系;其中HPV与癌组织细胞分类可能有一定的相关性,与癌细胞分化程度无明显相关性。更多还原

【Abstract】 Objective:To determine the relationship between human papillomavirus (HPV) and the genisis and histopathology of colorectal carcinoma (CRC).Methods:HPV DNA in 45 women with CRC and 35 controls was assayed by different primers-mediated polymerase chain reaction (PCR) to compare their pathological findings.Results:The general positive rate of HPV DNA in CRC was 42.2%, of HPV16 was 24.4%, which were significantly higher than that of control group (P<0.05).The positive rate of general HPV and HPV16 in high differentiation group were 41.2% and 17.6% respectively, those in the moderate, poor differentiation groups were 38.5%, 23.1% and 46.7%, 33.3%, respectively, but no significant difference on statistics (P>0.05).The general rate of HPV in signet-ring cell carcinoma was 50%, and was slightly higher than that of other lesions, such as mucoid papillary and tubular adenocarcinoma (P>0.05).HPV6/11 was only found in the tubular form (13.0%,P>0.05).HPV in signet-ring cell carcinoma and mucoid adenocarcinoma were all HPV16, the positive rate were 50% and 33.3% respectively. The rate in signet-ring cell carcinoma was significantly higher than that in papillary and tubular adenocarcinoma.Conclusion:HPV, especially HPV16, is associated with women CRC, HPV infection may relate to the tissue types of CRC, but there is no correlation between HPV and differentiation.更多还原
