输卵管妊娠放射介入治疗的临床分析

【摘要】 目的:探讨输卵管妊娠放射介入治疗的临床疗效及适应症。方法:选择38例输卵管妊娠患者,在X线影像引导下采用Seldinger技术插入导管超选择进入患侧子宫动脉内,一次性灌注MTX100mg,并使用明胶海绵颗粒栓塞治疗输卵管妊娠,观察其症状、体征、血β-HCG及B超结果的变化,随访卵巢内分泌功能情况。结果:34例输卵管妊娠经介入治疗成功,治愈率为89.5%。腹痛症状于介入治疗后3~14d内消失,阴道出血症状于介入治疗后2~10d内消失。血β-HCG于介入治疗后第3天50%病例明显下降;4周内97.1%病例转为阴性。B超检查盆腔液性暗区87%于介入治疗后1周内消失,附件包块于介入治疗后4周有29%包块缩小,71%包块消失。治疗后4周进行卵巢内分泌功能测定,FSH、LH及E2均在正常值范围。结果:早期输卵管妊娠患者采用放射介入治疗是有效的。

【Abstract】 Objective:To study the clinical therapeutical effects and indications of the radioactive interventional therapy for tubal pregnancy.Methods:38 patients with tubal pregnancy were treated with selective intubation, one-shot perfusion MTX 100 mg , combined with gelfoam particles embolization via uterine artery with Seldinger technique under X-ray fluoroscopic guidance. The changes of symptoms, signs, β-HCG in blood and results of B-ultrasonogram were observed and the state of ovarian endocrinal function was followed up.Results:34 cases were treated successfully, and the curative rate was 89.5%.After interventional therapy, abdominal pain disappeared within 3~14 days, vaginal bleeding released in 2~10 days. Theβ-HCG in blood of 50% of cases decreased obviously at the 3rd day and 97.1% of cases couldn’t be determined in 4 weeks. The liquid area opaca in the pelvic cavity of 87% of cases disappeared in 1 week, while mass of 29% of cases reduced and 71% of cases disappeared in 4 weeks through B utrasonogram. Ovarian endocrinal function determination found that FSH, LH and E 2 were within normal range in 4 weeks after treatment.Conclusion:The radioactive interventional therapy is effective for patients with early tubal pregnancy.
