氯胺酮、芬太尼、咪唑安定及异丙酚对TNF-α/IL-10平衡的影响

【摘要】 目的研究氯胺酮、芬太尼、咪唑安定及异丙酚对TNF-α/IL-10平衡的影响。方法动物按配伍组设计分为5组,分别为生理盐水对照组、氯胺酮组、芬太尼组、咪唑安定组和异丙酚组,麻醉1h后注射LPS,分别在麻醉前(T0)、注射LPS前(T1)、注射LPS后1h(T2)、2h(T3)、4h(T4)抽取静脉血用ELISA法测血清中TNF-α开始和IL-10的水平计算TNF-α/IL-10比值。结果除了异丙酚组外,其他三组TNF-α/IL-10在时点T2都达到峰值并下降,在注射LPS后氯胺酮、芬太尼和咪唑安定TNF-α/IL-10的比值明显低于对照组(P<0·01),异丙酚组则明显高于对照组(P<0·01)。结论从TNF-α/IL-10平衡的角度而言,氯胺酮、芬太尼和咪唑安定对于炎症反应初期细胞因子的平衡可能产生好的影响,而异丙酚则可能不利于细胞因子的平衡。

【Abstract】 Objective To study the effects of ketamine,fentanyl,midazolam and propofol on the balance of TNF-α/IL-10.Methods Methods animals were divided into 5 groups in randomized block design: saline group(control group),ketamine group,fentanyl group,midazolam group and propofol group. Every group was injected with LPS (20 μg/kg)after 1h transfusion.Blood samples were taken before anesthesia(T0), before LPS injection (T1),1 h(T2),2 h(T3),4 h(T4)after LPS injection,the serum TNF-α and IL-10 were detected by ELISA to calculate the value of TNF-α/IL-10.Results Except for propofol group,every group reached the peak then decreased at the point T2,the ratio of TNF-α/IL-10 in ketamine,fentanyl and midazolam group were significantly lower than that in control group(P<0.01),but that is significantly higher in propofol group than in control group(P<0.01).Conclusion In the view of balance of TNF-α/IL-10, ketamine, fentanyl and midazolam may be helpful to the cytokine balance in the initial stage of inflammatory reaction,but propofol could be harmful.
