利用EBCT进行颅外颈动脉血管造影的研究

【摘要】 目的:探讨利用电子束CT来实现对颈动脉分叉部血管造影的可行性,并建立颅外颈动脉EBCTA的扫描方式和方案。材料和方法:对24侧颈动脉随机分3组进行多层流动扫描(MSM)模式、连续容积扫描(CVS)模式及血管重建,获得颈动脉分叉部的时间-密度曲线(TDCs),并加以分析。结果:得出颈动脉EBCTA的参数和模式为:非离子型造影剂,总量80~90ml、3ml/s注射速率、延迟时间18~22s、扫描速度为层/0.1s、层厚1.5mm、由颈根部至颅底的CVS扫描方式。对有循环功能不足的患者应进行MSM预试验扫描来确定其准确的延迟时间。结论:EBCT进行颅外颈动脉血管造影完全可行,且省时,省造影剂,扫描范围长,更利于显示该区域的各种颈动脉疾病。

【Abstract】 Purpose:To prove the applicability and to set the protocol of EBCT angiograph y for scanning the carotid bifurcation.Materials and Methods: A randomized prosp ective study was designed within 12 patients. Twenty-four carotid arteries wer e scanned by using MSM(multi-slice mode)and then CVS(continuous volume sc anning) for the EBCTA. EBCT angiogrphic images were produced by MIP and SSD algo rithm. Results: We compared the results of these TDCs of MSM and acquisition ima ges of CVS, and determined the optimal acquisition parameters for EBCTA scanning the extracranial carotid artery as follow: nonionic contrast material 80~90 m/s, the injecting speed rate 3 m/s, delay time 18~22 s, scanning speed 0.1s for per slice, slice thickness 1.5mm, slice interval 1.5mm, using CVS mode from the neck root to skull base scan. For patients in weaken circulative function, the optimal delay time of scanning should be determined by TDCs obtained from t he individual flow test with MSM, independently. Conclusion: EBCT data displayed in MIP reconstruction showed excellent carotid arterial images.
