虚证与实证耳鸣的诱发性耳声发射信号特点研究

【摘要】 目的研究虚证与实证耳鸣患者的诱发性耳声发射信号特点,为耳鸣的临床辨证提供客观的参考指 标。方法对辨证为虚证(52耳)及实证(36耳)的耳鸣患者及耳科正常的对照组(56耳)进行畸变产物耳声发射 (DPOAE)、瞬态诱发性耳声发射(TEOAE)测试,记录两者重复性、反应强度与信噪比,比较其组间差异性。结果 虚证与实证耳鸣患者的TEOAE重复性与信噪比均有不同程度下降,实证DPOAE信噪比在1~2kHz可无下降,而 虚证耳鸣在此频段DPOAE信噪比下降。结论初步阐述了虚证与实证耳鸣诱发性耳声发射信号特点,此特点可 在一定程度上作为耳鸣辨证的客观依据。

【Abstract】 Objective To investigate the properties of evoked otoacoustic emissions specific for deficient and excessive syndromes of tinnitus. Methods DPOAE and TEOAE were tested among 52 ears with deficient syndrome of tinnitus ,36 ears with excessive syndrome of tinnitus and 56 healthy ears. In the experiments,the reproducibility,responsive amplitude and the ratio of signal to noise of the tests were carefully assayed and comparisons were made among the groups. Results Reduced reproducibility and the ratio of signal to noise in TEOAE were found at various degrees in the groups of tinnitus patients, either deficient or excessive syndrome. In the test of DPOAE,cases with an excessive syndrome of tinnitus showed no change in the ratio of signal to noise in the range of 1 to 2 kHz, while those with a deficient pattern of tinnitus showed a decreased ratio of signal to noise in this range of frequencies. Conclusions Different patterns of tinnitus will display some specific properties in the test of evoked otocccoustic emissions and these may provide some objective evidences for the pattern differentiation of tinnitus at a certain level.
