恩纳乳膏在腹腔镜手术病人静脉穿刺中的应用

【摘要】 目的观察恩纳乳膏用于腹腔镜手术病人术前静脉穿刺的镇痛效果,以及了解病人对该项护理操作的满意程度。方法将100例妇科腹腔镜手术病人随机分为实验组和对照组各50例。实验组选择以穿刺点为中心,沿静脉走向均匀涂上恩纳乳膏,60min后选择18号套管针进行静脉穿刺,对照组选择相同部位,相同型号套管针,按常规操作进行静脉穿刺;记录两组穿刺前、穿刺时、穿刺后5min、10min、20min、30min心率、血压情况,评估病人疼痛分值及满意度。结果两组病人穿刺术均成功,实验组疼痛评分明显低于对照组(均P<0.01);穿刺时对照组比实验组的血压、心率变化更明显(均P<0.01);根据静脉穿刺产生的疼痛程度评价其满意度,实验组满意度为100%,对照组为10%,两组比较,P<0.05,差异有统计学意义。结论恩纳乳膏可减轻手术病人术前静脉穿刺时的疼痛,此项护理操作得到病人的认同,具有临床推广应用价值。更多还原

【Abstract】 Objective To study the analgesia effect of ointment of eutectic mixture of local anesthetics (EMLA) on venipuncture before operation and the patients′ satisfaction degree with this nursing operation. Methods One hundred patients receiving gynaecology celioscope operation were randomly divided into two groups: the experiment group (n=50) and the control group (n=50). In the experiment group, EMLA (1.5g/10 cm2) was smeared symmetrically along vein’ vertical axes 5 cm and horizontal axes 3 cm (about 15 cm2) and pellicle was covered. After 60 minutes, patients in the experiment group were venipunctured with 18 gauge trocars. Patients in the control group were venipunctured according to routine operation with the same puncture site and the same type trocars. Before venipuncture, during venipuncture period and after venipuncture, HR,BP and arrhythmia frequency were recorded and patients’ pain was assessed. Results All venipuncture was made successfully in the two group patients. Pain in the experiment group was less than the control group significantly (P < 0.01) and arrhythmia frequency in the experiment group was fewer. BP and HR increased more significantly during the venipuncture period in the control group than in the experiment group (P < 0.05). Conclusion Ointment of EMLA can alleviate pain of the preoperative patients during venipuncture period. This nursing manipulation was accepted by the patients and has clinical application value.更多还原
