【摘要】 目的　研究和探讨OKC癌变的临床与组织病理学特征。方法　对2 2 6例OKC临床病理资料进行研究分析。结果　2 2 6例OKC中,6例OKC发生癌变。癌变病例临床特征是:①患者年龄多在4 0岁以上。②病变局部反复感染史,病史一般超过3年。③X线影像为直径3～6 .5cm多房透光区,边界较清,部分边界模糊。④术后多次复发。组织病理学特征是:①骨腔充满病变组织,类似实体肿瘤。②癌变病例全部为多囊性,单囊OKC未见癌变。③囊壁衬里上皮异常增生,侵犯性生长,可见异常核分裂,呈典型鳞癌结构。④局部骨组织破坏。癌组织可发生转移。结论　OKC癌变病例有明显的临床表现与组织病理学特征,多囊性OKC衬里上皮有较强的增殖活性和癌变潜能。更多还原

【Abstract】 Objective To study and reseach the clinical and histopathological features of the OKC with cancerous changes.Methods Data of 226 cases of OKC were sdudied.Monoclonal antibody against PCNA was used to check single and multiple cysts.Results Out of 226 cases,6 cases companied with cancerous changes.The clinical features of the cases with cancerous changes were:①Patients aged 40 more years.②Patients had recurrent local infection history.③X-ray images showed transparent areas which had diameter of 3～ 6.5cm.The image boundaries were clear but partial boundaries were not clear.Histopathological features were:①The bone cystic cavities were filled with lesion tissues,like solid tumor.②All cases with cancerous changes were multiple cysts not single cysts.Multiple cystic structures filled with keratotic material.③The lining epithelial cells proliferate abnormally.Partial proliferating cells presented typical squamous cells proliferated abnormally.Partial proliferating cells presented typical squamous cell carcinoma. Local bone was destroyed and metastasis may occur.Expressions of PCNA in lining epithelium of multiple cysts was stronger than that of single cysts.Conclusion There are evident clinical and pathological features in OKC with cancerous changes.The lining epithelium of multiple cysts has more proliferative activity and cancerous potentiality than that of single cysts.更多还原
