基质金属蛋白酶与类风湿关节炎等疾病的关系

【摘要】 目的:对基质金属蛋白酶的特性和调控表达机制及其与类风湿性关节炎的关系作一综述,为类风湿性关节炎患者的康复干预提供理论依据。资料来源:应用计算机检索Medline1975-01/2004-10文章,检索关键词为“matrixmetalloproteinase,emphysema,atherosclerosis,rheumatoidarthritis”,限定文章语言种类为English。资料选择:对资料进行筛选,选取与类风湿性关节炎相关的文献。纳入条件:①随机对照基础研究及临床实验。②非随机化同期对照研究。排除条件:①综述文献。②重复的同一研究。③Meta分析。资料提炼:共收集到30篇关于基质金属蛋白酶的随机和未随机实验,其中15个实验符合纳入标准,排除的16篇系重复的同一研究随机实验、Meta分析和综述文献。资料综合:从基础研究及临床实验方面对基质金属蛋白酶及基质金属蛋白酶12与类风湿性关节炎的关系进行分析。4个双盲实验(265例患者)用来评价基质金属蛋白酶及基质金属蛋白酶12与类风湿性关节炎的关系,与对照组相比,类风湿性关节炎患者的滑膜组织中巨噬细胞浸润增加,基质金属蛋白酶12表达量增高。结论:虽然基质金属蛋白酶12在类风湿性关节炎破坏中的具体作用机制尚不十分明确,但阻断单个的基质金属蛋白酶如阻断基质金属蛋白酶12有可能可以阻断关节破坏的进一步进展。

【Abstract】 OBJECTIVE:To review the characters and mechanism of regulating expression of matrix metalloproteinases(MMPs),and the relationship of MMPs with rheumatoid arthritis(RA) so as to support the rehabilitative intervention to RA patients theoretically. DATA SOURCES:A computer-based search of Medline for the related articles about rheumatoid arthritis published between January 1975 and October 2004 was performed by using theof "matrixmetalloproteinase,emphysema,atherosclero-sis,rheumatoid arthritis",and the language of the articles was limited in English. STUDY SELECTION:After screening to the articles,the contents of literatures about RA were selected.The inclusion criteria were for those ① randomized controlled basic studies and clinical experiments;②non-randomized controlled synchronous studies.The inclusion criteria were for those ①review;②repetitive studies; ③Meta analytic studies. DATA EXTRACTION:Totally 30 related articles to MMPs using random-ized and non-randomized experiment were collected.Of the 14 selected articles,14 experiments were accordant with the inclusion criteria,and 14 were excluded because they were the same studies wsing repetetive randomized experiments,Meta analytic stuclies or reviews. DATA SYNTHESIS:From the view of basic study and clinical experiment,the relationship of MMPs and MMP-12 with RA was analyzed.Four doble-blind controlled experiments in 265 patients were used to evaluate the relationship of MMPs and MMP-12 with RA.As compared with controls,the infiltration of histocytes in synovial tissues was more,and the expression of MMP-12 was higher in RA patients. CONCLUSION:The mechanism of MMP-12 in the destruction to RA is still unclear,but blocking single MMP,e.g.,MMP-12 may inhibit the destruction to joint.
