肥胖程度与血脂代谢及胰岛素抵抗的关系

【摘要】 目的:肥胖在高血压脑卒中的发生发展过程中起重要作用,探讨肥胖程度与胰岛素抵抗及血脂代谢的关系,对降低高血压脑卒中的发病有重要意义。方法:选择66例肥胖症患者,分为轻、中、重度肥胖组,体检正常者22例为对照组,均行空腹胰岛素、空腹血糖、空腹C肽、糖化血红蛋白及血清总胆固醇、三酰甘油、高密度脂蛋白胆固醇测定。结果:轻、中、重度肥胖组的空腹血糖、糖化血红蛋白、空腹胰岛素、空腹C肽、血清总胆固醇、三酰甘油水平明显高于对照组,与对照组比较,差异有显著性意义(P<0.05,P<0.01,P<0.001),且肥胖程度越重,差异越明显;重度肥胖组与轻、中度肥胖组比较也有显著性差异(P<0.01,P<0.05);而重度肥胖组高密度脂蛋白则较对照组明显降低(P<0.01)。结论:肥胖症存在高胰岛素血症,高胰岛素血症与高血糖并存,说明胰岛素的生物效应降低,存在胰岛素抵抗;同时脂质代谢紊乱可能也与胰岛素抵抗有关。

【Abstract】 AIM: Obesity plays an important role in the occurrence and development of hypertensive stroke. This paper aims to study the relationship of obese degree with insulin resistance and blood lipid metabolism, and it is significant to reduce the incidence rate of hypertensive stroke. METHODS: Fasting insulin (FINS), fasting blood glucose (FBG), fasting C peptide (FCP), glycosylated hemoglobin (HbA1C), and serum levels of total cholesterol (TC), triacyglycerol (TG) and high-density lipoprotein-cholesterol (HDL-C) were determined in 60 patients with obesity (mild, moderate and severe obesity group) and 20 normal physical examinees (control group). RESULTS: FBG, HbA1C, FINS, FCP and serum levels of TC and TG were significantly higher in the mild, moderate and severe obesity groups than in the control group (P< 0.05,P< 0.01, P< 0.001), and the severer the obese degree, the more obvious the differences, and those in the severe obesity group were significantly different from those in the mild and moderate obesity groups (P< 0.01, P< 0.05).HDL-C in the severe obesity group was markedly lower than that in the control group (P< 0.01). CONCLUSION: There is hyperinsulinism in patients with obesity, and hyperinsulinism is accompanied by hyperglycemia. It is suggested that the biological effect of insulin is decreased, and there is insulin resistance. Blood lipid metabolic disorder can be related with insulin resistance.
