2型糖尿病大血管病变患者血清肝细胞生长因子水平
Level of serum hepatocyte growth factor in type 2 diabetic macrovascular complications
【摘要】 目的:探讨2型糖尿病大血管病变患者血清肝细胞生长因子水平及与其相关因素分析。方法:选取2004-01/06中国医科大学附属第一临床医院152例实验对象,包括2型糖尿病合并大血管病变组44例,2型糖尿病无大血管病变组32例、无糖尿病大血管病变组36例以及40例健康体检者(正常对照组),行常规体质量、腰围、臀围、血压测量,测定空腹血糖、糖化血红蛋白、总胆固醇、三酰甘油、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇和尿酸,并采用酶联免疫吸附实验方法测定其血清肝细胞生长因子水平。结果用x珔±s表示,组间比较采用方差分析,血清肝细胞生长因子与各指标的相关性采用直线回归、多元逐步回归。结果:①2型糖尿病伴有或不伴有大血管病变患者及无糖尿病的大血管病变患者血清肝细胞生长因子水平比正常对照组明显增高,(1.745±0.475),(1.329±0.464),(1.413±0.565),(1.034±0.276)μg/L,P<0.05~0.01;无糖尿病大血管病变患者和2型糖尿病无大血管病变患者血清肝细胞生长因子水平基本相似;2型糖尿病伴有大血管病变患者血清肝细胞生长因子水平较2型糖尿病无大血管病变患者及无糖尿病大血管病变者明显增高(P<0.05)。②血清肝细胞生长因子和收缩压、空腹血糖、糖化血红蛋白及低密度脂蛋白胆固醇在P<0.05的水平上有显著的正相?
【Abstract】 AIM:To investigate the serum level of hepatocyte growth factor(HGF) in type 2 diabetic patients with macrovascular complications,and analyze the related factors. METHODS:From January to June 2004,152 cases,including type 2 diabetics with macrovascular complications(n=44),type 2 diabetics without macrovascular complications(n=32) and non-diabetic patients suffering from macrovascular disease(n=36),and 40 healthy physical examinees(normal control group) in the First Clinical Hospital of China Medical University received routine measurements of body mass,waist and hip circumferences and blood pressure,their fasting blood glucose,glycosylated hemoglobin A(HbA1C),total cholesterol(TC),triacylglycerol(TG),low density lipoprotein-cholesterol(LDL-C),high density lipoprotein-cholesterol(HDL-C) and uric acid were determined, the serum level of HGF was detected with enzyme-linked immunoadsordent assay(ELISA).The results were expressed as Mean±SD,and the intragroup comparison was carried out with the analysis of variance,the correlation between HGF and the clinical indexes was analyzed with linear regression and multiple stepwise regression. RESULTS:The HGF levels of type 2 diabetics with or without macrovascular complications and non-diabetic patients suffering from macrovcular disease[(1.745±0.475),(1.329±0.464),(1.413±0.565) μg/L] were obviously higher than that in the normal control group[(1.034±0.276) μg/L](P< 0.05 to 0.01),there was no significant difference in the HGF level between type 2 diabetics without macrovascular complications and non-diabetic patients suffering from macrovascular disease;It was markedly higher in type 2 diabetics with macrovascular complications than in type 2 diabetics without macrovascular complications and non-diabetic patients suffering from macrovascular disease(P< 0.05).②Serum HGF had significant positive correlations with systolic blood pressure, fasting blood glucose,HbA1C and LDL-C(r=0.56,0.37,0.41,0.64,P< 0.05),but had significant negative correlation with HDL-C(r=-0.27,P< 0.05). Multiple stepwise regression analysis was carried out with HGF as dependent variable,systolic blood pressure,and fasting blood glucose,HbA1C HDL-C and LDL-C as independent variables,HGF with LDL-C(r2=0.21) and systolic blood pressure(r2=0.19) were involved in the regression equation. CONCLUSION:Serum HGF level is significantly increased in type 2 diabetic patients, and the increase is more obvious in those with macrovascular complications.HGF can be used as a potential index for the severity of macrovascular complications in diabetes mellitus.HGF level is correlated with blood pressure,blood glucose and blood lipid.
