创伤能量与大鼠肢体深静脉血栓形成的相关性
Association between traumatic energy and deep venous thrombosis in rat limbs
【摘要】 目的:观察不同击打能量致骨折和非骨折以及仅固定肢体大鼠建立的隐大静脉血栓动物模型不同时相点的血栓发生率。方法:实验于2003-10/2004-03在昆明医学院外科总论实验室完成。将120只Wistar大鼠随机分为3组。以不同打击能量对大鼠双侧大腿近端外侧造成创伤建立创伤模型两组。①骨折固定组:瞬间击打能量为4.2J,经骨擦感证实股骨骨折后,双侧均行髋人字石膏固定。②创伤固定组:瞬间击打能量为2.4J,不造成股骨骨折,双侧均行髋人字石膏固定。③单纯固定组:仅行双侧髋人字石膏固定。造模后3,5,7,9d(每个时间点10只大鼠,20侧)对大鼠双侧隐大静脉进行组织学及血栓发生率观察,并对创伤能量与深静脉血栓形成的相关性进行分析。结果:纳入大鼠120只,实验过程死亡3只,进入结果分析117只。各组隐大静脉血栓发生率7d达高峰,骨折固定组最高(85%),创伤固定组次之(55%),单纯固定组最低(20%);7d时骨折固定组血栓形成数明显多于创伤组(32,18),创伤能量大的组深静脉血栓形成数多,相关系数=0.71,P<0.01。结论:大鼠隐大静脉血栓发生率与创伤能量大小呈正相关,并与肢体固定后局部静脉血流速度成减慢有关,血栓发生率高峰时间在伤后第7天。
【Abstract】 AIM:To investigate thrombosis rate of large saphenous veins in animal models e stablished or unfractured from the fractured rats by beating limbs with differen t strength and limb fixed rats. METHODS:The experiment was conducted in the laboratory for general introductio n,Department of Surgery,Kunming Medical College from October 2003 to March 2004. Totally 120 Wistar rats were divided randomly into fracture fixation,trauma fixa tion and single fixation groups.The trauma models were created by beating the do uble proximal lateral femurs of rats with different strength.①The momentary bea ting strength was 4.2 J for the fracture fixation group.The double thighs were f ixed with plaster onto the hips in a gable shape after they were verified to fra cture by rubbing bone.②The momentary beating strength was 2.4 J for the trauma fixation group.The double thighs were not fractured,but still fixed with plaster onto the hips in a gable shape.③The double thighs were only fixed with plaster onto the hips in a gable shape in the single fixation group.On the 3rd,5th,7th and 9th days after modeling,histological observation and thrombosis incidence de tection were performed in bilateral large saphenous veins of 20 thighs of 10 rat s at each time point.The correlation between traumatic energy and deep venous th rombosis(DVT) was analyzed. RESULTS:Three of the 120 rats died during experiment,so 117 rats finished the result analysis.The thrombosis incidence of large saphenous veins reached its pe ak at 7 days in all the groups:the highest in the fracture fixation group(85%), followed by trauma fixation group(55%) and the single fixation(20%).At 7 days, the incidence of DVT was higher in the fracture fixation group than in the traum a fixation group(32 vs 18).The higher the traumatic energy was,the higher the in cidence of DVT was.The correlation coefficient between traumatic energy and inci dence of DVT was 0.71,P< 0.01. CONCLUSION:The incidence of DVT in rats is not only positively correlated with the amount of traumatic energy,but also correlated with the flow velocity of lo cal venous blood in the fixed limbs.The highest incidence of DVT is on the postt raumtic 7th day.
