平肝潜阳针法对原发性高血压患者甲襞微循环的影响
Influence of calming liver and suppressing liver-yang hyperactivity on the microcirculation of nail fold in patients with primary hypertension
【摘要】 目的:观察平肝潜阳针法对原发性高血压的患者甲襞微循环的影响,探讨针刺治疗原发性高血压的机制。方法:随机选择泰山医学院附属医院针灸科2001-04/2002-10符合WHO原发性高血压诊断标准并且中医辩证为“肝阳上亢”的患者32例,按照平肝潜阳的原则,采用单纯针刺方法,取穴为双侧风池、太冲、涌泉、太溪,1次/d。分别于治疗前及治疗15d以后测量血压和观察甲襞微循环,并选取15名健康志愿者作为对照。结果:治疗15d后,32例原发性高血压患者的甲襞微循环及血压均有明显改善,血液流态积分、袢周状态积分、总积分治疗前后分别为(1.81±0.50,0.97±0.49)分,(1.11±0.50,0.70±0.45)分,(3.81±1.00,2.53±0.97)分,血压变化显效9例(28%),有效18例(56%),无效5例(16%),总有效率为84%(P<0.05),健康志愿者各项变化不明显(P>0.05)。结论:针灸疗法治疗原发性高血压,其机制可能与改善微循环状态,从而减小血管外周阻力有关。
【Abstract】 AIM:To observe the effect of calming liver and suppressing liver-yang hyperactivity on the microcirculation of nail fold in patients with primary hypertension,and investigate the mechanism of acupuncture in treating primary hypertension. METHODS:Thirty-two patients, who were in accordance with the WHO diagnostic criteria for primary hypertension and had the Chinese differentiating symptoms of hyperactivity of the liver-yang,were selected randomly from the Department of Acupuncture and Moxibustion,Affiliated Hospital of Taishan Medical College from April 2001 to October 2002.They were treated with simple acupuncture according to the principle of calming liver and suppressing liver-yang hyperactivity at the acupoints of bilateral Fengch’ih,T’aich’ung,Yungch’uan and T’aihsi,once a day. Blood pressure was measured and nail fold microcirculation was observed before and 15 days after treatment respectively.Fifteen healthy volunteers were taken as controls. RESULTS:After the 15-day treatment, nail fold microcirculation and the blood pressure of the 32 cases of primary hypertension were all improved obviously,the score of blood flow status,peri-ansae score and total score before and after treatment were(1.81±0.50, 0.97±0.49),(1.11±0.50, 0.70±0.45) and(3.81±1.00, 2.53±0.97) respectively,the change of blood pressure was significantly effective in 9 cases(28%), effective in 18 cases(56%) and invalid in 5 cases(16%), the total effective rate was 84%(P< 0.05),while there were no obvious changes in the healthy controls (P >0.05). CONCLUSION:The mechanism of acupuncture in treating primary hypertension is probably related with the improvement of microcirculation and the reduction of the resistance of small blood vessel.
