【摘要】 目的:探讨A型肉毒毒素局部注射能否有效改善脑卒中患者下肢痉挛,观察其安全性。方法:1998-12/2002-01赤峰市第三医院神经内科收治的脑卒中患者,应用下肢肌肉局部注射国产A型肉毒毒素治疗25例,每个患者每次选择3～5块肌肉,每块肌肉总的注射剂量为50IU。所有患者同时接受康复治疗。注射前、注射后2周、4周按改良式Ashworth量表(modifiedAshworthscale,MAS)评定肌张力;同时根据步行功能的分类来评定步行功能。结果:整体比较,注射后2,4周各肌群肌张力降低。髋内收肌注射前、注射后2,4周MAS为2.30±0.63,1.15±0.89,0.92±0.64,差异有非常显著性意义(F=20.36,P<0.001),绳肌、腓肠肌、股直肌注射后肌张力均显著降低。结论:下肢肌肉局部注射A型肉毒毒素可以明显改善脑卒中患者的痉挛步态。月国更多还原

【Abstract】 AIM:To explore whether local injection of type A borulinus toxin is an effective and safe treatment for lower spastic after stroke. METHODS: Totally 25 patients with stroke were selected from the Department of Neurology of the Third Hospital of Chifeng from December 1998 to January 2002.The patients received local lower intramuscular injection of homemade A borulinus toxin. Three to five muscles were chosen every time for each patient and the injected dose was 50 IU in all for each muscle. All patients received rehabilitation therapy at the same time. The muscle tones were assessed before, and 2, 4 weeks after injection with the modified Ashworth scale(MAS);the walking ability was assessed according to the classification of walking function. RESULTS: The total muscle tones decreased 2,4 weeks after injection.MAS of hip adductor before and 2,4 weeks after injection were 2.30±0.63,1.15±0.8,0.92±0.64,and there was significant differences (F=20.36,P< 0.001). The muscle tone of hamstring muscle,gastrocnemius muscle and rectus femoris reduced significantly after injection. CONCLUSION: The results of the present study indicate that local injection of type A borulinus toxin into the muscles of lower limb results in an improvement in spastic gait in patients with cerebral apoplexy.更多还原
