定量肌电图运动单位电位多项参数对肌萎缩侧索硬化症患者的评估价值
Multiple motor unit potential parameters of quantitative electromyography for ev aluating the patients with amyotrophic lateral sclerosis
【摘要】 目的:分析肌萎缩侧索硬化(amyotrophiclateralsclerosis,ALS)神经传导和F波的变化以及单纤维肌电图(SFEMG)的异常,探讨定量肌电图运动单位电位(motorunitpotential,MUP)多项参数对ALS患者的评估价值。方法:福建省立医院神经内科收治的ALS患者60例和体检的健康者55例对照组,对两组对象进行定量肌电图检查和常规运动及感觉神()经传导测定。结果:MUP参数对ALS诊断以面积、时限、大小指数最敏感;多相波敏感性较差;自发电位敏感性小于MUP参数。ALS运动末端潜伏期(DML)延长和复合动作电位CMAP)波幅下降,F波出现率下降与肌萎(缩有明显关系,ALS在SFEMG表现为颤抖增宽和出现阻滞,纤维密度明显增高。结论:肌电图、单纤维肌电图、神经传导、F波检查可在无明显临床症状时即发现下运动神经元损害,对ALS早期诊断、鉴别诊断和病情预后评估有重要价值。
【Abstract】 AIM:To analyze the changes of nerve conduction velocity(NCV),F wave and abnor malities of single fiber electromyography(SFEMG) of amyotrophic lateral sclerosi s(ALS) patients,and study the quantitative electromyography(EMG) of patients wit h amyotrophic lateral sclerosis(ALS),evaluate the value of different parameters of motor unit potential(MUP) for evaluating ALS. METHODS:Totally 60 ASL patients and 55 healthy subjects(control group) were se lected from the Department of Neurology,Fujian Provincial Hospital.Quantitative EMG,and motor nerve conduction velocity(MNCV) and sensory nerve conduction veloc ity(SNCV) were estimated in two groups. RESULTS:Among the five MUP parameters:area,duration and size index(SI) present ed the higher sensitivity for the diagnosis of ALS,while sensibitity of polyphas ic wave was lower,spontaneous activity was less sensitive than MUP parameters.It was found that there was strong correlation between prolongation of distal moto r latency and descent of the amplitude of compound action potential,low F wave ratio and myatrophy of ASL patients.The SFEMG of ALS patients also presented the increase of jitter,block and fiber density(FD). CONCLUSION:EMG,SFEMG,NCV and F wave can determine the damage of lower motor n euron without any obvious clinical symptoms.There is significant value for the e arly diagnosis and differential diagnosis of ALS and prognostic evaluation.
