妊娠期妇女阴道念珠菌菌种分析

【摘要】 目的:对妊娠期妇女的阴道念珠菌菌种进行研究,明确妊娠期阴道念珠菌的检出率和菌种的分布,了解妊娠期阴道念珠菌与相应的临床症状有无相关性。方法:随机对627例门诊妊娠妇女阴道分泌物进行念珠菌培养和鉴定,比较不同孕期阴道念珠菌检出率和菌种分布的差异,分析各孕期白念、非白念构成比对孕妇临床症状的影响。结果:妊娠期妇女阴道念珠菌的检出率为31.26%,以白色念珠菌最为多见,占62.44%,其次是光滑念珠菌(26.90%)、克柔氏念珠菌(4.06%)和热带念珠菌(2.54%);早、中、晚孕期阴道念珠菌的检出率分别为34.82%、28.25%和30.56%,均以白念和光滑念珠菌为主;随着妊娠的进展,念珠菌阳性孕妇中有症状者逐渐增多,有症状的孕妇白念珠菌比率逐渐减少,而非白念珠菌的比率逐渐增加。结论:妊娠期阴道念珠菌的检出率较非孕妇女明显增加,以白念珠菌为主,光滑念珠菌为非白念的主要菌种;妊娠各期念珠菌检出状况没有显著差异;各孕期白念和非白念的构成比对妊娠妇女的临床症状有规律性影响。

【Abstract】 Objective:To study on colonization and the distribution of candida species during pregnancy,determine correlation between candida species and clinical symptoms.Methods: Vaginal swabs isolated from 627 pregnant women were incubated on the sabouraud(SAB) agar.Candida species were identified and compared among different trimesters.How distribution of C.albicans and nonalbicans influences clinical symptoms were analyzed.Results:The colonization of candida species in pregnant women was 31.26%.C.albicans was the most frequently(62.44%) isolates followed by C.glabrata(26.90%),C.krusei (4.06%) and C.tropicalis(2.54%).The colonization of candida species during the first,the second,the third trimesters were 34.82%、28.25%,30.56% and dominated by C.albicans.Along with pregnant evolvement,symptom patients and nonalbicans increased while C.albicans species decreased.Conclusion:The candida species colonization during pregnancy is very common.C.albicans and C.glabrata are major species.The colonization and distribution of candida species among three trimesters is identity.Constitution of C.albicans and nonalbicans has disciplinarian influence to clinical symptoms.
