食管鳞状细胞癌中VEGF与TIMP-2、MVD、树突状细胞的关系及对浸润转移的影响

【摘要】 目的探讨与VEGF相关的TIMP2表达、血管形成、肿瘤免疫异常对食管鳞癌浸润转移的影响。方法运用原位杂交和免疫组织化学方法检测46例食管鳞癌组织中VEGF蛋白和mRNA的表达、TIMP2的表达、CD34标记的微血管密度、S100标记的肿瘤浸润性树突状细胞的密度。结果食管鳞癌组织中VEGF蛋白阳性表达率为58.7%(27/46),其阳性表达与浸润深度、转移状态有关;TIMP2表达与分化、浸润深度有关,VEGF与TIMP2之间没有相关关系;VEGF表达阳性组MVD(50.71±12.16/单位面积)显著高于阴性组(31.74±17.93/单位面积);食管鳞癌中S100+树突状细胞平均密度与分化程度、浸润深度的有关,VEGF表达与树突状细胞密度之间存在显著的负相关性(r=-0.462)。结论VEGF蛋白表达具有促进食管鳞癌浸润转移的作用,免疫组化方法较原位杂交更适合临床作为判断食管鳞癌浸润转移潜能的手段;TIMP2可抑制食管鳞癌的浸润,其表达与VEGF表达之间无相关关系;MVD值可作为判断食管鳞癌浸润、转移及预后的参考指标;VEGF影响鳞癌组织中树突状细胞的密度,进而可能影响宿主的抗肿瘤免疫能力和肿瘤的浸润转移过程。

【Abstract】 Objective To investigate the effects of factors which are associated with VEGF, such as TIMP-2 expression, microvessel density and tumor immunity abnormality,on the invasion and metastasis of esophageal squamous cell carcinoma. Methods The expression of VEGF, MVD, dendritic cells, tissue inhibitor-2 of matrix metallo-proteinase in forty-six specimens of esophageal squamous cell carcinoma were examined by immunohistochemistry staining (S-P method) and in situ hybridization,using anti-VEGF monoclonal antibody, anti-CD34 monoclonal antibody, anti-S-100 polyclonal antibody, anti-TIMP-2 polyclonal antibody and labeled VEGF mRNA probes respectively. Results The rates of VEGF protein expression were 58.7% (27/46) and its expression had significant associations with the invasion and metastasis of the esophageal squamous cell carcinoma. The expression of TIMP-2 protein had significant association with the grade and invasion of the esophageal squamous cell carcinoma, and no association between the expression of the VEGF and TIMP-2 protein was found. The MVD in VEGF positive expression tumor(50.71±12.16/Area Unit) were significant higher than those in VEGF negative expression tumor(31.74±17.93/Area Unit). There were great difference of the density in dendritic cell in tumor of different grade and metastasis situation, and there was a close negative relation between the expression of VEGF and the density in dendritic cell in esophageal squamous cell carcinoma(R=-0.462). Conclusion VEGF protein can promote the invasion and metastasis of esophageal squamous cell carcinoma, and detecting VEGF protein with immunohistochemistry is better than with in situ hybridization when comes to the clinical application to assess the potential ability of invasion and metastasis of the tumor. The TIMP-2 protein may inhibit the invasion of esophageal squamous cell carcinoma and there is no relationship between the expression of TIMP-2 and VEGF. The MVD of esophageal squamous cell carcinoma is associated with the invasion,metastasis, expression of VEGF protein, but not with differentiation, which showed that MVD can be used as a reference method to check the invasion, metastasis,prognosis of carcinoma. The expression of VEGF protein affected the density of TIDC, so may affect the ability of anti-tumor immunity, as well as the invasion and metastasis of the esophageal squamous cell carcinoma.
