侧块钢板在颈椎单开门手术中的应用

【摘要】 目的:总结侧块钢板螺钉系统在颈椎后路单开门手术中的应用经验。方法:对2001年2月至2004年6月间共12例行颈椎单开门并侧块钢板内固定的患者资料进行回顾研究。12例均获得随访,平均随访时间9个月(6~15个月)。12例患者中颈椎病(3节及3节段以上椎间盘突出)3例,黄韧带骨化症(OYL)1例,后纵韧带骨化症(OPLL)2例,发育性颈椎管狭窄6例。患者术前存在颈痛,有程度不等的脊柱不稳。手术为单开门椎板成型、关节突间植骨融合及相应节段的侧块钢板螺钉内固定术。结果:术后随访6至15个月,12例患者中颈痛明显改善者10例(占83.3%)。12例患者关节突间见骨小梁生成,融合率100%。1例(2枚螺钉)出现螺钉松动。结论:颈椎后路侧块钢板螺钉系统能提供较稳定的固定,能有效改善颈椎不稳的症状,有利于颈椎开门处的愈合。严格掌握手术适应证及技术,颈椎后路钢板固定是治疗颈椎不稳的一种有效的方法。
【Abstract】 Objective To summarize the clinical experience of applying posterior lateral mass plates fixation in the open-door laminoplasty of cervical. Methods 12 cases posterior lateral mass plate fixation in the open-door laminoplasty patients were studied retrospectively. All cases were followed up. The average time of follow-up was 9 months (range from 6 to 15 months). 12 cases included 3 cases of cervical spondylotic myelopathy, 1 cases of ossification of yellow ligament (OYL), 2 cases of ossification of posterior longitudinal ligament (OPLL), 6 cases of congenital cervical stenosis. Results Neck pain was ameliorated in 10 of 12 patients. Solid arthrodesis was achieved in 12 patients. Complications associated with placement of the screws included screw loosening in 1 case. Conclusion Posterior lateral mass plating is highly effective, provide stable fixation, improve the fusion and relieve neck pain.
