星形细胞肿瘤CT灌注成像与肿瘤微血管相关性的研究
The Correlative Study Between CT Perfusion Imaging and Microvascular Density of Astrocytoma
【摘要】 目的分析星形细胞肿瘤CT灌注参数与肿瘤微血管密度(MVD)的相关性;探讨CT灌注成像对星形细胞肿瘤术前分级的价值。方法星形细胞肿瘤患者18例:术前2~8d常规CT及CT灌注检查,术后病理标本常规组织切片及免疫组织化学检查,单克隆抗CD34抗体标记血管。结果18例星形细胞肿瘤患者中,低级星形细胞肿瘤7例;高级星形细胞肿瘤11例。星形细胞肿瘤的脑血流量(CBF)、脑血容量(CBV)和表面通透性(PS)均高于正常脑白质,高级星形细胞肿瘤的CBF、CBV和PS高于低级星形细胞肿瘤。肿瘤的CBF、CBV和PS与肿瘤微血管密度(MVD)均呈正相关。结论星形细胞肿瘤的CT灌注参数与肿瘤MVD呈正相关,CT灌注有助于星形细胞肿瘤的术前准确分级,CT灌注可为脑肿瘤和非肿瘤性病变的鉴别诊断提供有价值的参考信息。
【Abstract】 Objective To!analyze the relationship between the CT perfusion imaging and microvascular density (MVD) of astrocytomas,and to evaluate the value of CT perfusion on grading of astrocytomas before operation.Methods 18 patients with astrocytoma underwent CT examinations 2d~8d before operation. CT examination including: scout view, plain scan, enhanced dynamic scan and enhanced scan. The perfusion 2 software was used for the dynamic data analysis; The cerebral blood flow(CBF),cerebral blood volume(CBV),mean transit time (MTT)and permeability surface(PS) mapping images were acquired. The HE and immunohistochemistry staining of the specimen were acquired after operation. The MVD of the tumors were calculated with the Weidner’s technique.Results There were 7 cases of low-grade (WHO, grade Ⅱ) astrocytoma tumors and 11cases of high-grade(WHO,grade Ⅲ~Ⅳ)astrocytoma tumor. The values of CBF, CBV and PS of the astrocytoma tumors were higher than that of white matters; the values of CBF, CBV and PS of the high grade astrocytoma tumors were higher than that of low grade astrocytoma tumors. There were positive correlation between the tumor MVD and tumor CBF, CBV, and PS. Conclusion CT perfusion imaging is helpful for astrocytoma grading before operations and can be used to distinguish the low-grade astrocytoma and high-grade astrocytoma. It may offer useful informations on differential diagnosis between the tumorous lesions and non-tumorous lesions.
