股骨头缺血性坏死组织形态学特点的实验研究
An experiment study of Histology & Morphology of Ischemic Necrosis of Femoral Head
【摘要】 目的探讨股骨头缺血坏死的病理变化。方法采用22例人工股骨头(全髋关节)置换术后完整股骨头标本,分为坏死组(14例)、正常组(8例),并按Ficat分期将坏死股骨头分为Ⅰ、Ⅱ期组和Ⅲ、Ⅳ期组,观察、测定、分析外观改变、组织形态学变化、髓腔脂肪细胞染色及计数、最大脂肪细胞平均直径、骨小梁面积、空骨陷窝计数。结果坏死组较正常组,骨质松脆、易于凿切,髓腔脂肪细胞增大、增多,骨小梁面积减小,空骨陷窝增多(P<0.05),且随病情发展,以上变化更加明显,其统计学意义显著(P<0.01)。结论脂质代谢紊乱、骨质疏松是股骨头缺血性坏死的重要病理变化之一.
【Abstract】 Objective To explore the pathological alteration of Ischemic Necrosis of Femoral Head (INFH) in order to provide data for diagnosis, and experimental and theoretical basis for the way of the prevention and treatment of INFH, clinically. Methods The specimens of INFH from Femoral Head Replacement (FHR) or Total Hip Replacement (THR) were divided into two groups: INFH group and Normal group, the latter caused by trauma injury wound. In addition, the specimens of INFH, in the meantime, were divided according to Ficats standard. The alteration of histology & morphology were observed. The number of fat cell in the femoral head marrow (FHM), the empty osteocytes trabecuae and the length of diameter of the biggest fat cell were measured. Bone trabeculae was squared. Results Compared with normal group, the changes of INFH group, obviously, took place: increase and enlargement of fat cell in the FHM, decrease in the number of the osteocytes and the bone trabeculae. Conclusion The alteration of INFH was due to fat degeneration and osteoporosis.
