【摘要】 目的　探讨靶基因 CEAm RNA在食管癌患者外周血中的表达及与肿瘤微转移的关系。方法　应用巢式 -反转录聚合酶链反应 (Nested- RT- PCR)检测 5 0例无远处转移、10例有远处转移的食管癌患者 ,10例食管良性病变和 10例健康成人外周血中癌胚抗原CEAm RNA基因的表达。结果　食管癌良性病变者和健康成人的外周血中均未检测出 CEAm RNA阳性细胞。5 0例无远处转移、10例有远处转移的食管癌患者外周血中 CEAm RNA阳性率分别为 30 .2 % (16 / 5 0 )、80 .0 % (8/ 10 ) ,P<0 .0 1;食管癌 期与 a、 b、 期者阳性率比较差异有显著性(P均 <0 .0 1) ,而其他各期之间阳性率比较差异无显著性 (P>0 .0 5 ) ;不同程度病理分化者间阳性率比较差异无显著性 (P>0 .0 5 )。结论　食管癌患者临床早期已有癌细胞微转移。应用 Nested- RT- PCR检测外周血中CEAm RNA靶基因有助于早期诊断。更多还原

【Abstract】 Objective To explore the relationship between the expression of carcinoembryonic antigen mRNA target gene and micrometastasis in the peripheral blood of patients with esophageal carcinoma.Methods Nested-reverse-transcription-polymerase chain reaction(nested-RT-PCR)was used to detect the expression of carcinoembryonic antigen mRNA in the peripheral blood of 50 esophageal carcinoma patients without distal metastasis(experimental group),10 esophageal carcinoma patients with distal metastasis,10 benign esophageal disease patients and 10 healthy adults.Results CEAmRNA was not detected in benign esophageal disease patients and healthy adults.in the peripheral blood of 50 esophageal carcinoma patients without distal metastasis,the expression of CEAmRNA was positive in 16 (30.2%)cases.CEAmRNA was detected in 8 of 10(80%)cases in the esophageal carcinoma patients with distal metastasis.the positive rates in stageIV were significantly higher than stageⅡa、stageⅡb and stageⅢ(P<0.01).But there were no significant difference in stageⅡa、stageⅡb and stageⅢ(P>0.05).The differentiation of pathological type were no significant difference in the positive rates(P>0.05).Conclusions In the early stage,the cancer cells have already metastasized.Nested-RT-PCRwas used to detect the expression of CEA mRNA in the peripheral blood,it is helpful for early diagnosis of tumor micrometastasis.更多还原
