胃癌MHC-I、MHC-II类抗原表达的研究
Expressions of MHC-I and MHC-II antigen in the stomach cancer
【摘要】 目的:探讨组织相容性复合体I、II(MHC-I、MHC-II)类抗原表达与胃癌发生发展的关系。方法:应用流式细胞仪检测胃癌、癌旁组织、慢性胃炎及正常胃粘膜组织MHC-I、MHC-II类抗原的表达,并结合临床病理进行分析。结果:MHC-I、MHC-II类抗原在胃癌组织的表达分别为26.58±11.21、29.85±10.84,显著低于癌旁组织(72.36±25.59,63.89±18.46)、慢性胃炎(78.51±21.26,66.55±23.24)、胃溃疡(74.88±24.29,61.36±27.71)及正常胃粘膜组织(76.13±16.22,66.32±16.53)(P均<0.01),而癌旁组织、慢性胃炎、胃溃疡及正常组织之间无明显差异(P均>0.05);高、中、低分化胃癌组织MHC-I、MHC-II类抗原表达分别为(40.32±19.56,31.67±14.38)、(29.51±17.16,30.29±13.78)、(10.83±5.51,28.71±11.64),MHC-I在各组比较差异显著(P均<0.05),MHC-II比较无明显差异(P均>0.05);伴远处淋巴结转移较局部转移和无转移MHC-I、MHC-II类抗原表达明显降低,后二者也有明显差异(P均<0.05);伴肠上皮化生及不典型增生胃粘膜MHC-I、MHC-II类抗原表达较单纯慢性胃炎明显降低(P均<0.05);肠上皮化生较轻、中度不典型增生无明显差异(P>0.05);而重度不典型增生较轻、中度不典型增生及肠上皮化生胃粘膜MHC-I、MHC-II类抗原表达明显降低(P均<0.05)。结论:MHC-I、MHC-II类抗原表达降低在胃癌发生发展中起重要作用。
【Abstract】 Objective: To investigate the relationship between the expression of MHC;I and MHC;II antigen and the occurrence and differentiation of the stomach cancer. Methods: The expressions of MHC;I and MHC;II antigen in stomach carcinoma , paracarcinomic tissues ,chronic gastitis and gastric ulcer were examined by flow cytometry. Results:The expressions of MHC;I and MHC;II antigen were lower in tumor tissues than those in paracarcinomic and other tissues( P<0.01),and the expression of MHC;I antigen was lower in the low different iated than that in the middle and high tissues( P<0.05 ), and the moderate was lower than the high. The expressions of MHC;I and MHC;II antigen were significantly lower in tumor tissues with distant lymph node transference than those with local transference, and those expressions in the stomach mucosa tissues with intestinal metaplasia and dysplasia were also dramatically lower than those with chronic gastritis( P<0.05). Conclusions : The low expressions of MHC;I and MHC;II are closely related to the occurrence and transference of stomach cancer.
