【摘要】 [目的]观察风湿性心脏病瓣膜病变合并心房纤颤(房颤)病人行瓣膜替换加微波消融术后的疗效。[方法]对7例风湿性心脏病瓣膜病变合并持续性心房纤颤史12个月至44年的患者,在行瓣膜置换手术的同时行微波心内膜消融术,观察术毕、术后24 h、出院时的心律情况。[结果]术毕7例全部房颤消失,代之为节性心律,其中4例需临时起搏;术后24h房颤复发1例;出院时房颤复发2例。全组均无栓塞等并发症,无术后死亡。[结论]在风湿性心脏病瓣膜病变合并房颤患者行瓣膜替换手术同时做微波消融操作方便,可将房颤心律转为窦性心律,但对有左房血栓、心房壁钙化的病例,效果不理想。更多还原

【Abstract】 [ Objective ] To observe the effect of valve replacement combined with microwave ablation for rheumatic valvular heart disease complicated with atrial fibrillation. [ Methods ] Seven patients with rheumatic valvular heart disease combined with continuous atrial fibrillation for 12 months to 44 years received endocardial microwave ablation when having valve replacement. The cardiac rhythm of the patients was compared at the time point after operation, 24 hours after operation and at discharge time. [ Results ] At the time point after operation, auricular fibrillation disappeared and rhythmic rhythm occurred in 7 patients, and 4 need temporary pacing; atrial fibrillation recurred in one patient 24 hours after operation and in 2 patients at discharge time. No complications such as infarction occurred and no death was found. [Conclusion] Valve replacement combined with microwave ablation can promote the transformation of sinus rhythm from atrial rhythm on patients with rheumatic valvular heart disease complicated with atrial fibrillation, but has no obvious effect on patients with left atrial thrombus, calcification of atrial wall.更多还原
