气道肿瘤经纤维支气管镜介入微波治疗的临床观察

【摘要】 目的:探讨经纤维支气管镜介入微波治疗气道肿瘤的操作要点、适应证及临床疗效。方法:对60例中央型支气管肺癌及3例气道良性肿瘤患者采用经纤维支气管镜介入微波行热疗。结果:该疗法对60例肺癌患者完全缓解22例(367%),部分缓解28例(467%),其中腔内肿块型总有效率931%(27/29)。3例气道良性肿瘤均获显效。阻塞性肺炎总好转率885%(46/52),肺不张总有效率852%(23/27)。对改善气喘、发热症状疗效显著,有效率分别为891%(41/46)、938%(45/48)。结论:气道良性肿瘤及伴支气管阻塞的中央型肺癌是经纤维支气管镜介入微波热疗的良好适应证,它能为气道肿瘤患者提供除手术、放疗及化疗外,又一安全、实用且有效的辅助治疗手段。

【Abstract】 OBJECTIVE:To evaluate the performance keys,indications and clinical effects of microwave thermotherapy on airway tumor under bronchofiberoscope.METHODS:The therapeutic procedure was performed on sixty cases of central type bronchogenic pulmonary carcinoma and three cases of benign airway tumor.RESULTS:The complete and partial responces of sixty lung cancer cases to the method were in 22 cases (36.7%) and 28 ones (46.7%),respectively.The total effective rate of the endobronchial neoplasm type lung cancer cases was 93.1%(27/29).Three benign airway tumor cases all gained complete effect.The total improved rates achieved in obstructive pneumomia and atelectasis were 88.5%(46/52) and 85.2%(23/27) respectively.The total effective rates of improving dyspnea and fever symptom were 89.1%(41/46) and 93.8%(45/48),respectively.CONCLUSIONS:Benign airway tumor and central type lung cancer accompanied by airway obstruction are good indications of the microwave thermotherapy under bronchofiberoscope.It is a safe, practical and effectvie adjuvant method for the treatment of airway tumor besides operation,radiotherapy and chemotherapy.
