人工合成材料修复齿槽裂的实验研究

【摘要】 目的齿槽裂的骨修复是先天性唇腭裂序列治疗的一个重要阶段,现多采用髂骨松质骨移植的方法。将膜引导组织再生术和人工合成材料应用于兔的人工齿槽裂模型,观察骨愈合的效果,以期寻找一种新的治疗方法。方法取18只成丰新西兰白兔,随机分为三组。A组作为对照组,制作人工齿槽裂模型后直接缝合;B组在模型骨缺损的两端用膨体聚四氟乙烯(e-PTFE)膜覆盖;C组则应用人工合成材料倍骼生植入骨缺损,e-PTFE膜覆盖。于21日分别处死动物。结果 X线片示A组骨边缘清晰,无阻光阴影;B组有少量阻光阴影;C组有大片阻光阴影。组织切片示A组骨缺损区软组织生长,成骨细胞可见,有大量新生血管;B组成骨细胞数量较多,已有骨组织生成;C组有明显条纹状骨小梁生成,其间有倍骼生颗粒。经图像分析和统计学处理,三组两两比较后P值均<0.001,之间均有显著性差异。结论引导组织再生术和人工合成材料均能明显引起骨生成,两者结合应用时效果更好,为齿槽裂骨修复提供了一种新的治疗方法。

【Abstract】 Objective Bone reconstruction of alveolar cleft has been developed to the stage of systematic treatment in the patients with congenital cleft lip and palate. Nowdays,cancellous bone of iliac has been grafted to the defect of alveolar cleft. Observing the effects of bone healing in the model of alveolar cleft of rabbits by membrane of guided tissue regeneration and grafting of PerioGlas and it will lead to a new treatment. Methods Eighteen New Zealand white rabbits were used in this study and were divided in three groups in random. As a contrast,suturing the mucoperiosteal flap directly in group A when the animal models of alveolar cleft were made. Using guided tissue regeneration(GTR) with e-PTFE membrane in group B. In group C, using both the e-PTFE membrane and PerioGlas of synthetic materials. Results In X-ray,group A showed ridge of bone defect clearly and hadn’t the shadom; group B showed a little shadom and group C showed a lot of shadom. Histologically, group A showed a lot of roft tissue in bone defect,a few osteoblasts and a lot of new capillary vessel;group B showed a great number of osteoblast and bone had formatted resently; group C showed obviously bone formation and PerioGlas were existed among the bone. Comparing the three groups with each other,there were excellent difference among the three groups. Conclusion There were obvious bone reconstruction both in using GTR and synthetic materials. The effects were more better when the two methods were used together than one method only. We consider that it is a worthy method for bone reconstruction of alveolar cleft.
