【摘要】 目的　探讨我院两年来医院感染致病菌的临床分布特点及耐药现状 ,为临床用药提供参考。方法　用API鉴定系统及K -B纸片扩散法对 2 0 0 2～ 2 0 0 3年我院检出的 1 680株医院感染细菌的分布特点及药敏结果进行统计学分析。结果　在检出的医院感染细菌中占临床分离率前几位的菌种从高到低依次为 :真菌、大肠埃希氏菌、铜绿色假单胞菌及金黄色葡萄球菌 ,且这几种细菌均高于全国网络资料 ;从药敏结果分析显示 :医院感染菌对常用抗生素耐药情况严重 ,革兰氏阳性菌对青霉素、红霉素、氯苄青、复方新诺明、苯唑青的耐药率达 75 .2 %～94.1 % ,革兰氏阴性菌对氨苄青霉素、复方新诺明、头孢唑林、庆大霉素的耐药率达 65 .1 %～ 97.6% ,与临床用药呈正相关。产ESBLs菌株呈逐年上升 ,对各种抗生素的耐药率也显著增高 ,使此类感染的治疗十分困难。结论　院内细菌耐药情况不容乐观 ,应积极采取有效措施控制细菌耐药性的产生及蔓延。更多还原

【Abstract】 Objective To explore the clinical distributed features and drug-resista nt conditions of disease-causing bacteria in the hospital in the last two years for the reference to clinical drug administration. Methods The distributed featu res and drug allergy of 1680 strains of infections germs detected were statistic all y analyzed by API identifying system and K-B paper strip method during the year s of 2002 to 2003. Result The first few strains of hospital disease-causing germ s were fungus, escherilchia coli, pseudomonas aeruginosa and staphy lococ cus aureus respectively. The drug allergy analysis showed that the tolerance rat e of Gram-pos itive bacteria against penicillin, erythromycin, ampicillin,compound sulfamethox azole,oxacillin came to 75.2%～94.1%,and that of Gram-negative bacteria against a mpicillin,compound sulfamethoxazole,cefazolin,gentamycin reached 65.1%～ 97 .6%. Th e development of ESBLs strains tends to increase year by year. The tolerance rat e against a variety of antibiotics also rose remarkably, so that the infection of this kind was hard to be cured. Conclusion?The bacterial drug-fast conditions cannot be optimistic and effective measures should be positively taken to preven t bacterial drug tolerance from happening and spreading.更多还原
