呼吸机相关性肺炎50例临床分析

【摘要】 目的:分析呼吸机相关性肺炎(VAP)的临床特点,探讨VAP发生的可能原因和药敏结果。方法:回顾性分析近10月我院ICU142例机械通气2d以上发生VAP50例的临床资料。结果:VAP的发生率为35.2%,其发生可能与气管插管的方式、插鼻胃管、使用肌松镇静药、制酸药和广谱抗生素等有关。病原学检出率100%,其中G-菌71.25%,G+菌28.75%,主要菌株为铜绿假单胞菌、耐酶金黄色葡萄球菌(MRSA)、鲍曼不动杆菌等,且多为耐药菌株。结论:VAP的发生率、死亡率高,一旦发生VAP,应尽早根据药敏结果选用敏感抗生素。

【Abstract】 Objective:To investigate the possible agent and therapeutic effect of ventilator-associated pneumonia (VAP). Methods:We analyze retrospectively the clinical data of 50 cases with VAP from 142 patients admitted to intensive care unit(ICU) of our hospital who required mechanical ventilation for more than 2 days. Results:The morbidity of VAP was 35.2%. The agent may relate to the mode of tracheal intubation?gastric tube interpolation?the use of skelaxin?sedatives?antacids and broad-spectrum antibiotic. Pathogens were isolated from every case by sputum cultivation. Gram-negative bacteria were 71.25%, Gram-positive bacteria were 28.75%. The pseudomdnas aeruginosa? methicillin resistant staphylococcus aureus (MRSA)?baumanii ect. Most of them were drug resistant strain. Conclusion: The morbidity and mortality of VAP was high. Favorite antibiotic should be adopt early according to the result of susceptibility test once VAP occurs.
