带蒂腓肠神经营养血管逆行岛状皮瓣修复小腿下段软组织缺损的临床应用

【摘要】 目的探讨腓肠神经营养血管蒂逆行岛状皮瓣的临床应用效果。方法应用腓肠神经营养血管蒂逆行岛状皮瓣修复小腿下段软组织缺损14例,皮瓣切取范围4cm×3.5cm~24cm×16cm。术中利用皮瓣远端腓肠神经与受区浅表神经的远端架接。结果12例皮瓣完全成活,2例皮瓣远端部分坏死,经换药后愈合。经6~12个月随访,皮瓣感觉神经外形良好。皮瓣传导感觉平均二点分辨率为1.5cm,随访期内未出现失神经性溃疡。结论腓肠神经营养血管蒂逆行岛状皮瓣血供丰富,不牺牲重要血管神经,手术操作简单,可较好地修复小腿下部软组织缺损。

【Abstract】 Objective To analyze the results of clinic application of distal base nutritious sural nerve island flap repairing nerve nutrition. Methods The posterior island flap of pedicle with sural nerve and its nutrient vessels was applied to repair soft tissue injuries in leg in 14 cases. The rang of flap was 4cm×3.5cm~24cm×16cm. The far port of the sural nerve was connected with the far port of the wound broken nerve. Results 12 flaps survived completely , partially necrosed in 2 flaps, but it would be healed after dressing change. The patients were followed up for 6~12 months and all flaps had fine appearance and function. 2-PD was about 1.5cm.There were not any case having canker by the end of the follow up time. Conclusion Sural nerve flap has more reliable blood supply and does not need to damage important vessels and nerves. It is simple and ease to be done, and provides an effective method for repairing the defect of soft tissue on the leg.
