【摘要】 目的　探讨小梁切除术中改良缝合 ,滤过泡自然形成 ,术后不按摩眼球的远期效果。方法　对 15例(17只眼 )不同类型的青光眼行小梁切除 ,术中巩膜瓣改良缝合 ,缝线位于小梁切口后缘 0 .5 mm～ 1m m的巩膜瓣两侧 ,并形成前房 ,试压滤过检查 ,术后不按摩眼球 ,随访 1～ 5年。结果　 17只眼术后 7天内眼压 :5～ 8mm Hg者 9只眼 ,9～ 14 mm Hg者 6只眼 ;半年后 :5～ 8m m Hg者 2只眼 ,9～ 14 mm Hg者 9只眼 ,15～ 18mm Hg者 6只眼。滤过泡 :7天内 型 17只眼 ,7天后～半年 型 13只眼 , 型 4只眼 ;半年后 : 型 11只眼 , 型 6只眼。结论　小梁切除术中巩膜瓣改良缝合 ,滤过泡自然形成 ,术后长期不用按摩眼球 ,远期效果好 ,安全、有效。更多还原
