【摘要】 目的　 探讨上尿路结石所致积水肾脏中 ,肾小叶间动脉血流阻力指数 (RI)与残存肾功能以及术后肾功能恢复程度的关系。方法　 对 5 5例经手术治疗的上尿路结石所致单侧肾积水病人 ,术前、术后放射核素扫描检查肾脏功能 ,按肾脏小叶间动脉RI值的不同 ,对比分析RI值与积水肾的肾功能损害程度以及术后恢复程度间的关系。结果　 肾脏小叶间动脉RI值为 <0 .6 5、0 .6 5～0 .75和 >0 .75时 ,患肾功能分别为总肾功能的 (38.5 1± 8.87) %、(2 4 .31± 7.71) %和 (6 .39±4 .5 1) % ;小叶间动脉RI <0 .6 5时 ,术后肾功能恢复快 ,术后 12个月患肾功能可以达到或接近正常 ;RI >0 .75时 ,患肾功能受损严重 ,解除梗阻后恢复较慢 ,术后 12个月患肾功能仅为总肾功能的(10 .6 4± 5 .4 4) %。结论　 肾小叶间动脉RI是判断上尿路结石所致积水肾实质损害程度的可靠指标 ,RI >0 .75时 ,肾功受损严重 ,术后恢复差 ,可以作为积水肾切除的一项参考指标。更多还原

【Abstract】 Objective To explore the relationship betwee n the intrarenal resistive index(RI) and the remanet and postoperative recovery of renal function in kidneys with hydronephrosis caused by calculi in upper urinar y tract.Methods In 55 patients who were treated operatively for t he hydronephrosis caused by calculi in upper urinary tract,the preoperative dama g degree and the postoperative recovery degree of the renal function were analyz ed according to different grades of intrarenal RI.Results The cor responding intrarenal RI were less than 0.65 ?0.65 to 0.75 and more than 0.7 5,th e percentage of the renal function in the kidney with hydronephrosis to the tota l renal function were (38.51±8.87)%?(24.31±7.71)%和(6.39±4.51)% respectively .The renal function recovered quickly in patients with RI less than 0.65.While t he renal function recovered slowly and not distinctly in patients with RI more t han 0.75.Conclusion The intrarenal RI is reliable in the estimation of the damag degree of renal fun ction in patients with hydronephrosis caused by calculi in upper urinary tract.W hen the RI is more than 0.75,the renal function has a serious damage and cannot have a obvious recovery after operation,and it can be a index for the nephrectom y in hydronephrosis.更多还原
