【摘要】 目的探讨总结儿童双侧肾上腺肿瘤的诊治经验。方法结合文献对6例儿童双侧肾上腺肿瘤病例资料进行分析讨论。结果本组6例儿童双侧肾上腺肿瘤均手术切除。病理检查嗜铬细胞瘤(APA)8处,高醛固酮症(IHA)4处。术后症状消失,恢复良好,随访1-3年未见复发。结论术前明确肾上腺肿瘤的性质是手术成功的前提。应根据不同类型肾上腺疾病的影像学特点,选择合理的影像学方法进行定位和定性诊断。性质不确定、无手术史的小肾上腺肿瘤可考虑将电视腹腔镜作为治疗的首选方法。经皮穿刺化学消融术治疗肾上腺肿瘤有临床的合理和可行性。更多还原

【Abstract】 Objective To explore as well as summarize the experience of diagnosis and therapy for bilateral adrenal gland tumor in children. Methods Medical history, physical examination, laboratory tests, X-ray and CT image of 6 children affected from the bilateral adrenal gland tumor were reviewed with conbined the recent the disease literature. Results 6 cases of bilateral adrenal gland tumors were removal by using operation,pathologic sections showed that at 8 sites the tumors lesion belongs to pheochromocytoma,other 4 lesion sites were aldosterone-producing adenoma. Findings of followed up for 1～3 years provided these tumors without recurrence,recovery good and all symptoms disappearanced. Conclusions Based on our experience,the surgery success key is that clear-cut diagnosis of adrenal gland tumors before the operation,according to image characteristics of different types of the adrenal gland tumors,the surgeon can select adequate image techniqesuch as X-ray,CT and MRI etc to locate and diagnose the disease. The telomerase activity may help to distinguish between benign and malignant tumors. Video laparoscopy is the first choice for small tumors of properties still uncertain and without operation history. Percutaneous chemoablation therapy is useful for some cases of adrenal gland tumors.更多还原
