卡维地洛对充血性心力衰竭患者一氧化氮、内皮素-1及肿瘤坏死因子-α的影响

【摘要】 目的:探讨充血性心力衰竭(CHF)患者血浆内皮素-1(ET-1)、一氧化氮(NO)和肿瘤坏死因子-α(TNF-α)的变化及卡维地洛的干预作用。方法:将58例CHF患者随机分为2亚组:常规治疗亚组30例,给予强心、利尿、扩血管等抗常规治疗;卡维地洛治疗亚组28例,在常规治疗的基础上加用卡维地洛。采用放射免疫法和比色法分别测定58例CHF患者和20例健康者(正常对照组)血ET-1、NO和TNF-α浓度。结果:①CHF组血ET-1、NO和TNF-α水平明显高于正常对照组(P<0.05);②CHF组的2亚组患者治疗后心功能均有改善,卡维地洛治疗亚组治疗后ET-1、NO和TNF-α下降显著。结论:ET-1、NO及TNF-α参与心力衰竭的发生发展,卡维地洛可降低心力衰竭患者ET-1、NO及TNF-α水平,改善心力衰竭时血管内皮系统的功能紊乱。

【Abstract】 Objective:To study the level changes of plasma endothelin-1(ET-1) , nitric oxide (NO), and tumor necrosis factor-α(TNF-α) and the intervention role of dilatrend in patients with congestive heart failure (CHF). Method:Fifty-eight cases of heart failure were randomized into A group of 30 cases and B group of 28 cases. A group were given cardiotonic, diuretic and extending blood vessel drugs; B group were given dilatrend 6.25mg twice a day in the duration of two months, besides the medications administered by A group. NO, ET-1 and TNF-α levels were measured in 58 patients with CHF and 20 healthy subjects served as normal controls. Result: The serum NO, ET-1 and TNF-α levels in patients were much higher than those in normal subjects (all P<0.05). Both of the therapies were effective in improving the symptoms of CHF, but only the patients treated with dilatrend lowered serum NO, ET-1 and TNF-α (P<0.05). Conclusion:NO, ET-1 and TNF-α are involved in the pathophysiologic process of heart failure. Dilatrend may decrease the levels of NO, ET-1 and TNF-α in patients with heart failure and improve the dysfunction of endothelial system.
